摘 要:目的探究血清胱抑素C(Cys-C)水平与慢性阻塞性肺疾病(COPD)发展、肺功能变化的相关性。方法选取2020年4月至2021年10月本院收治的300例COPD住院患者作为研究对象,根据病情严重程度分为急性加重期组、稳定期组与高危期组,每组100例,另选取本院100名健康志愿者作为对照组,应用生化分析方法测定3组血清Cys-C水平,并进一步分析Cys-C与肺部功能参数的相关性。结果急性加重组、稳定组、高危组血清Cys-C水平明显高于对照组(P<0.05),急性加重组与稳定组血清Cys-C水平比较差异无统计学意义,急性加重组与稳定组血清Cys-C水平均明显高于高危组(P<0.05)。Spearman相关分析显示,COPD患者血清Cys-C水平与第1秒用力呼气容积(FEV1)占预计值的百分比(FEV1%pred)、FEV1与用力肺活量(FVC)的比值(FEV1/FVC)、COPD疾病临床测试量表(CAT)评分呈负相关(P<0.05)。多元线性回归分析显示,COPD患者血清胱抑素C水平与FEV1%pred、CAT评分成反比(P<0.05),性别、体重指数(BMI)和吸烟状况不是影响胱抑素C水平的显著因素。结论胱抑素C可能为肺组织破坏和COPD严重程度的潜在生物标志物。Objective To investigate the correlation between serum Cystatin C(Cys-C)level and the development of chronic obstructive pulmonary disease(COPD)and the change of pulmonary function.Methods 300 COPD inpatients admitted to our hospital from April 2020 to October 2021 were selected as the research subjects,and they were divided into acute exacerbation group,stable group and high-risk group according to the severity of the disease,with 100 cases in each group,and 100 healthy volunteers in our hospital were selected as the control group.Serum Cys-C levels were determined by biochemical analysis,and the correlation between Cys-C and pulmonary function parameters was further analyzed.Results Serum Cys-C levels in acute exacerbated group,stable group and high-risk group were significantly higher than at in control group(P<0.05).There was no significant difference in serum Cys-C between acute exacerbated group and stable group.The serum Cys-C levels in acute exacerbated and stable groups were significantly higher than those in high-risk groups(P<0.05).Spearman correlation analysis showed that serum Cys-C levels in COPD patients were negatively correlated with orced expiratory volume in one second/forced vital capacity as a percentage of predicted(FEV1%pred),FEV1/forced vital capacity(FEV1/FVC)and COPD assessment test(CAT)scores(P<0.05).Multiple linear regression analysis showed that serum cystatin C level in COPD patients was inversely proportional to FEV1%pred and CAT score(P<0.05).Gender,body mass index(BMI)and smoking status were not significant factors affecting cystatin C level.Conclusion Cystatin C may be potential biomarker of lung tissue destruction and COPD severity.
