摘  要:目的探讨鼠神经因子联合前列地尔治疗老年糖尿病周围神经病变(DPN)的疗效及对患者神经传导速度的影响。方法选取2020年1—9月本院收治的141例DPN患者作为研究对象,按照随机数字表法分为单用组(n=71)与联用组(n=70)。单用组采用前列地尔治疗,联用组采用鼠神经生长因子联合前列地尔治疗,比较两组治疗前后运动神经传导速度(MNCV)及感觉神经传导速度(SNCV)、神经症状评分(NSS)及临床疗效。结果治疗前,两组MNCV中正神经与腓总神经、SNCV中正神经与腓总神经传导速度比较差异无统计学意义;治疗后,两组MNCV中正神经与腓总神经、SNCV中正神经与腓总神经传导速度均快于治疗前,且联用组快于单用组,差异有统计学意义(P<0.05)。治疗前,两组NSS评分比较差异无统计学意义;治疗后,两组NSS评分均低于治疗前,且联用组低于单用组,差异有统计学意义(P<0.05)。联用组治疗总有效率为97.14%,高于单用组的87.32%,差异有统计学意义(P<0.05)。结论鼠神经因子联合前列地尔治疗DPN疗效显著,能有效改善患者周围神经病变程度,提高运动和感觉神经传导速度。Objective To investigate the efficacy of rat nerve growth factor combined with alprostadil in the treatment of senile diabetic peripheral neuropathy(DPN)and its effect on nerve conduction velocity.Methods 141 patients with DPN admitted to our hospital from January to September 2020 were selected as the research subjects,and they were divided into single group(n=71)and combined group(n=70)according to the random number table method.The single group was treated with alprostadil,and the combined group was treated with rat nerve growth factor combined with alprostadil.The motor nerve conduction velocity(MNCV),sensory nerve conduction velocity(SNCV),neurological symptom assessment score(NSS)and clinical efficacy were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in the conduction velocity of MNCV median nerve and common peroneal nerve,SNCV median nerve and common peroneal nerve between the two groups;after treatment,the conduction velocity of MNCV median nerve and common peroneal nerve,SNCV median nerve and common peroneal nerve in the two groups were faster than those before treatment,and the combined group was faster than the single group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in NSS scores between the two groups;after treatment,the NSS scores of the two groups were lower than those before treatment,and the combined group was lower than the single group,the difference was statistically significant(P<0.05).The total effective rate of the combined group was 97.14%,which was higher than 87.32%of the single group,the difference was statistically significant(P<0.05).Conclusion Rat nerve factor combined with alprostadil is effective in the treatment of DPN,and can effectively improve the degree of peripheral neuropathy and motor and sensory nerve conduction velocity.
