RT-3DE技术联合CMRI评估中重度PAH患者右室收缩功能的价值观察

摘　　要：目的 探究实时三维超声心动图(RT-3DE)按术联合心脏核磁共振(CMRI)评估中重度肺动脉高压(PAH)患者右室收缩功能的价值。方法 将2017年8月至2019年8月本院的中、重度患者PAH患者各15例作为研究组Ⅰ、研究组Ⅱ,将30例同期的健康体检者作为对照组。研究组Ⅰ、研究组Ⅱ患者均行常规超声心动图[指标包含右心室心肌做功指数(RIMP)、右心室面积变化分数(RVFAC)、三尖瓣收缩期峰速(S’)、三尖瓣收缩期位移(TAPSE)]与RT-3DE、CMRI检查[指标包含右心室心肌做功指数(RIMP)、右心室面积变化分数(RVFAC)、三尖瓣收缩期峰速(S’)、三尖瓣收缩期位移(TA PSE)],对照组接受常规超声心动图及RT-3DE检查。分析RT-3DE联合CMR评估中重度PAH患者右室收缩功能的价值。结果 研究组Ⅰ、研究组Ⅱ与对照组RVESV、TAPSE、RVEDV、RVFAC值均差异明显(P<0.05)。研究组Ⅰ、研究组Ⅱ患者TA PSE、RIMP、RVFAC、S’与RVEF的相关系数分别为0.568、0.712、0.757、0.406(P<0.05),对照组、研究组Ⅰ、研究组Ⅱ中分别以RVFAC、RIMP、S’与RVEF相关性最好(P<0.05);研究组中RT-3DE与CMRI检测结果 差异均不明显(P>0.05)。结论 RT-3DE技术联合CMRI可较准确评估中重度PAH患者的右室收缩功能,为临床评估提供重要依据。Objective To explore the evaluation value of real-time three-dimensional echoca rdiogra phy(RT-3 DE)combined with cardiac magnetic resonance imaging(CMRI) for right ventricular systolic function in patients with moderate and severe pulmonary arterial hypertension(PAH).Methods A total of 25 patients with moderate PAH and 25 patients with severe PAH in the hospital were enrolled as studyⅠ and study Ⅱ groups between August 2017 and August 2019,respectively.A total of 30 healthy controls during the same period were enrolled as control group.The patients in study Ⅰ group and study Ⅱ group underwent routine echocardiography [right ventricular index of myocardial performance(RIMP),right ventricular fractional area change(RVFAC),systolic peak velocity of tricuspid valve(S’),tricuspid annular plane systolic excursion(TAPSE)],RT-3 DE and CMRI examination(RIMP,RVFAC,S’,TAPSE),while control group underwent routine echocardiography and RT-3 DE.The evaluation value of RT-3 DE combined with CMRI for right ventricular systolic function in patients with moderate to severe PAH was analyzed.The differences in RVESV,TAPSE,RVEDV and RVFAC among study Ⅰ group,study Ⅱ group and control group were statistically significant(P<0.05).The correlation coefficients between TAPSE,RIMP,RVFAC,S’ and RVEF in study Ⅰ group and study Ⅱgroup were 0.568,-0.712,0.757 and 0.406,respectively(P<0.05).RVFAC,RIMP and S’ had the best correlation with RVEF in study Ⅰ group,study Ⅱ group and control group,respectively(P<0.05).There was no significant difference in the examination results between RT-3 DE and CMRI in study group(P>0.05).Conclusion RT-3 DE combined with CMRI can more accu rately evaluate right ventricular systolic function in patients with moderate and severe PAH.which can provide important basis for clinical evaluation.
