MR动态增强TIC曲线联合表观弥散系数在乳腺良恶性病变中的应用价值

摘　　要：目的 探讨MR动态增强TIC曲线联合表观弥散系数(ADC)鉴别乳腺良性和恶性病变的价值。方法 回顾性分析24例应用MR动态增强进行乳腺扫描并都获取了病理结果 的患者。以病理为金标准,计算准确率、敏感性、特异性、阴性预测值及阳性预测值。结果 病理结果 显示14例浸润性导管癌,2例混合型浸润癌,1例导管内癌,1例神经内分泌癌,4例纤维腺瘤,1例导管内乳头状瘤,1例乳腺腺病。乳腺MR动态增强TIC曲线联合ADC诊断乳腺癌的准确率为91.6%,敏感性94.4%,特异性83.3%,阴性预测值83.3%,阳性预测值94.4%。结论 MR动态增强TIC曲线联合ADC鉴别乳腺良恶性病变极具临床价值,可极大提高乳腺癌的诊断准确率。Objective To explore the value of MR dynamic enhancement TIC curve combined with apparent diffusion coefficient(ADC) in differentiating the breast benign and malignant lesions.Methods retrospective analysis was performed on 24 patients who underwent breast scan with MR dynamic enhancement and ADC and obtained pathological results.The accuracy,sensitivity,specificity,negative predictive value and positive predictive value were calculated using pathology as the gold standard.Results pathological results showed 14 cases of intraductal carcinoma,2 cases of mixed intraductal ca rcinoma,1 case of intraductal ca rcinoma,1 case of neu roendocrine ca rcinoma,4 cases of fibroadenoma,1 case of intraductal papilloma,and 1 cases of breast adenosis.The accuracy rate of diagnosis of breast cancer with MR dynamic enhanced TIC curve combined with ADC was91.6%,sensitivity 94.4%,specificity 83.3%,negative predictive value 83.3%,positive predictive value94.4%.Conclusion MR dynamic enhanced TIC curve combined with ADC in differentiating benign and malignant breast lesions is of great clinical value and can greatly improve the diagnostic accuracy of breast cancer.
