摘　　要：目的 探讨气腹CT在肠粘连的诊断及分级诊断中的应用价值。方法 回顾性分析2016年1月1日至2020年6月1日间行人工气腹CT检查的患者,按照气腹CT的影像学表现将肠粘连的严重程度分为四级,并与手术结果 进行对照,采用Kappa检验分析气腹造影CT的诊断结果 与手术结果 的一致性。结果 气腹造影CT对肠粘连的诊断敏感度为98.18%,特异度为100%,准确度为98.27%,气腹造影CT对肠粘连的诊断结果 与手术结果 具有高度一致性(kappa=0.848,P<0.05)。气腹造影CT对肠粘连的分级诊断结果 与手术结果 具有高度一致性(kappa=0.797,P<0.05)。结论 人工气腹造影CT不仅能够准确诊断肠粘连,并且能够对于肠粘连的严重情况进行分级诊断,这为明确肠粘连患者的诊断以及术前对粘连严重程度进行评估、降低手术风险、避免再次粘连具有重要意义。Objective To explore the value of the application of the CT scan of artificial pneumoperitoneum in the diagnosis and graded diagnosis of intestinal adhesions.Methods A retrospective analysis of patients who underwent CT examination of artificial pneumoperitoneum from January 1,2016 to June 1,2020 were collected.According to the imaging findings of the pneumoperitoneum CT,the severity of intestinal adhesions was divided into four levels.And the imaging results were compared with the surgical results.The kappa test was used to analyze the consistencyof the diagnosis results between the imaging results and the surgical results.Results The diagnostic sensitivity of the pneumoperitoneum CT for intestinal adhesions was 98.18%,the specificity was 100%,and the accuracy was 98.27%.The diagnostic results of CT pneumoperitoneum for intestinal adhesions were highly consistent with the surgical results(kappa=0.848,P<0.05).The graded diagnosis of intestinal adhesions by CT with pneumoperitoneum was highly consistent with the surgical results(kappa=0.797,P<0.05).Conclusion The CT scan of artificial pneumoperitoneum can not only accurately diagnose intestinal adhesions,but also make graded diagnosis of the severity of intestinal adhesions.And This is of a great significa nce to cla rify the diagnosis of patients with intestinal adhesions and to evaluate the severity of adhesions before surge ry,reduce the risk of surgery,and avoid re-adhesion.
