自拟定眩汤治疗前庭神经炎(痰浊中阻证)的疗效观察

摘　　要：目的观察定眩汤治疗前庭神经炎(痰浊中阻证)的疗效。方法患者91例随机分为对照组和治疗组。对照组采用常规激素加甲磺酸倍他司汀,治疗组在对照组的基础上加用自拟定眩汤,治疗7、14 d后进眩晕障碍量表(DHI)评分、冷热试验CP值、vHIT的增益、白细胞介素-6(IL-6)、中性粒细胞/淋巴细胞比值、血小板/淋巴细胞比值、中医症状积分的变化。结果治疗后治疗组的DHI评分、冷热试验CP值、IL-6、中性粒细胞/淋巴细胞比值、血小板/淋巴细胞比值、中医症状单项积分、中医症状积分低于对照组(P<0.05);治疗组vHIT的增益高于对照组(P<0.05);治疗组的临床总有效率为91.30%,优于对照组的75.56%(P<0.05)。结论定眩汤联合激素+倍他司汀对前庭神经炎(痰浊中阻证)治疗效果优于单纯使用激素加倍他司汀。Objective:To evaluate the efficacy of Dingxuan Decoction on vestibular neuritis(pattern of phlegmturbidity obstructing middle-jiao).Methods:A total of 91 patients were randomly assigned to the control group and observation group.The control group was given hormone and betahistine mesylate,and the observation group was given Dingxuan Decoction on the basis of the control group.The DHI score,caloric test CP value,gain of vHIT,interleukin-6(IL-6),neutrophil/lymphocyte ratio,platelet/lymphocyte ratio,and changes in TCM symptom scores after 7 and 14 days of treatment of the two groups were compared.Results:After 14 days of treatment,the DHI score,CP value,IL-6,neutrophil-to-lymphocyte ratio,platelet-to-lymphocyte ratio,single score of TCM symp⁃toms and TCM symptom score of the treatment group were lower than those of the control group(P<0.05).The gain and total clinical effective rate of vHIT in the observation group were higher than those of the control group(P<0.05).The total clinical effective rate was 91.30%in the observation group and 75.56%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Dingxuan Decoction combined with hor⁃mone and betahistine has better efficacy in treating vestibular neuritis(pattern of phlegm-turbidity obstructing mid⁃dle-jiao)than hormone and betahistine alone.
