真实世界丹参川芎嗪注射液治疗脑血管疾病人群的临床用药特征研究

摘　　要：目的:分析真实世界丹参川芎嗪注射液治疗脑血管疾病患者的临床用药特征。方法:对来源于全国21家三级甲等医院信息系统的4400例脑血管疾病患者,采用频数及关联规则分析方法对数据进行分析。结果:在使用丹参川芎嗪注射液治疗的脑血管疾病患者中,平均年龄67.62岁,男性居多(共2533例,占比62.95%),脑血管疾病西医诊断以脑梗死(共2501例,占比49.14%)最为常见,主要单种联合用药有乙酰水杨酸(共2479例,占比3.12%)、蛋白水解物(共1733例,占比2.18%)、阿托伐他汀(共1636例,占比2.06%),2种联合用药以银杏叶口服制剂+蛋白水解物最为常见;单种中药联合单种西药类别以活血化瘀剂+血液代用品和灌注液为主;单种中药联合2种西药类别以活血化瘀剂+心脏病治疗药+血液代用品和灌注液最常见;单种中药联合3种西药类别则以活血化瘀剂+心脏病治疗药+血液代用品和灌注液+口腔疾病治疗药最为常见。结论:丹参川芎嗪注射液的适用人群以中老年人居多,临床上常与活血化瘀剂、血液代用品和灌注液、心脏病治疗药、口腔疾病治疗药等联合使用,药物临床使用剂量与药物说明书存在出入,临床医生应谨慎对待,本研究为丹参川芎嗪注射液的临床使用提供参考依据。Objective:To analyze the clinical characteristics of Danshen Chuanxiongqin injection in the treatment of cerebrovascular diseases in the real world.Methods:Frequency analysis and association rule analysis were performed with the information of 4400 patients with cerebrovascular diseases from the information systems of 21 grade A tertiary hospitals in China.Results:The patients treated with Danshen Chuanxiongqin injection had an average age of 67.62 years and were mainly male(2533 patients,accounting for 62.95%).Cerebral infarction was the most common cerebrovascular disease(2501 patients,49.14%).The main single drugs for combined application with Danshen Chuanxiongqin injection were acetylsalicylic acid(2479 patients,3.12%),protolysate(1733 patients,2.18%),and atorvastatin(1636 patients,2.06%).The main two drugs for combined application were oral preparation of Ginkgo Folium+protolysate.The common combination of single Chinese medicine+single western medicine was the agent for activating blood to remove stasis+blood substitute and perfusion fluid.The common combination of single Chinese medicine+two western medicines was the agent for activating blood to remove stasis+western medicine for heart disease+blood substitute and perfusion fluid.The common combination of single Chinese medicine+three western medicines was the agent for activating blood to remove stasis+western medicine for heart disease+blood substitute and perfusion fluid+western medicine for oral disease.Conclusion:Danshen Chuanxiongqin injection was mainly applicable for the elderly and often combined with the agent for activating blood to remove stasis,blood substitute and perfusion fluid,medicine for heart disease,and medicine for oral disease in clinical practice.The clinical application dosage of Danshen Chuanxiongqin injection was different from the drug instructions,so clinicians should be cautious about it.This study provides a reference for the clinical use of Danshen Chuanxiongqin injection.
