摘　　要：目的探讨直接抗病毒药物(DAA)联合高效抗逆转录病毒疗法(HAART)治疗对丙型肝炎病毒(HCV)合并人类免疫缺陷病毒(HIV)感染患者肝脏功能和免疫功能的影响。方法选取2017年1月至2020年6月于本院抗病毒门诊就诊的60例HCV并HIV感染患者,按照随机信封法分为对照组和观察组,每组30例。对照组接受聚乙二醇干扰素α-2a、利巴韦林联合HAART方案治疗,观察组接受索磷布韦、利巴韦林联合HAART方案治疗。比较两组治疗前后肝功能指标和T淋巴细胞亚群检测情况及不良反应发生情况。结果治疗前,两组肝功能指标谷丙转氨酶(ALT)、谷草转氨酶(AST)比较差异无统计学意义;治疗后,两组ALT、AST水平均明显低于治疗前,且观察组低于对照组(P<0.05)。治疗前,两组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)比较差异无统计学意义;治疗后,两组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)均明显高于治疗前,且观察组CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组(P<0.05)。观察组不良反应发生率为10.00%,低于对照组的33.33%(P<0.05)。结论DAA联合HAART治疗能改善HCV合并HIV感染患者肝脏功能,促进免疫功能恢复。Objective To investigate the effect of direct antiviral agent(DAA)combined with highly active antiretroviral therapy(HAART)on liver function and immune function in patients with hepatitis C virus(HCV)and human immunodeficiency virus(HIV)infection.Methods A total of 60 patients with HCV and HIV infection who visited the antiviral clinic of our hospital from January 2017 to June 2020 were selected and divided into control group and observation group according to the random envelope method,with 30 cases in each group.The control group received peginterferonα-2a and ribavirin combined with HAART regimen,while the observation group received sofosbuvir and ribavirin combined with HAART regimen.The liver function indexes,T lymphocyte subsets before and after treatment and the incidence of adverse reactions were compared between the two groups.Results Before treatment,there was no significant difference in liver function indexes ALT and AST between the two groups,after treatment,ALT and AST levels in two groups were significantly lower than before treatment,and the observation group was lower than the control group(P<0.05).Before treatment,there was no significant difference in CD4^(+),CD8^(+),CD4^(+)/CD8^(+)between the two groups;after treatment,CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in the two groups were significantly higher than before treatment,and the levels of CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05).The incidence of adverse reactions in observation group was 10.00%,which was lower than 33.33%in control group(P<0.05).Conclusion DAA combined with HAART can improve liver function of HCV patients with HIV infection and promote the recovery of immune function.
