摘 要:目的 探究经皮椎弓根钉内固定联合椎体成形术治疗中老年骨质疏松性单节段胸腰椎压缩性骨折患者的临床效果。方法 回顾性分析2019年3月至2020年4月本院收治的60例中老年骨质疏松性单节段胸腰椎压缩性骨折患者的临床资料,根据治疗术式的不同进行分组,将应用传统开放椎弓根钉复位内固定术治疗的患者作为对照组,应用经皮椎弓根钉内固定联合椎体成形术治疗的患者作为观察组,每组30例。比较两组临床指标、治疗前后疼痛程度、功能障碍、伤椎前缘高度、伤椎后凸Cobb角。结果 观察组手术时间、切口长度均短于对照组,术中出血量少于对照组(P<0.05)。治疗后,两组视觉模拟评分法(VAS)评分、腰椎Oswestry功能障碍指数(ODI)评分、伤椎后凸Cobb角均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组伤椎前缘高度高于治疗前,且观察组高于对照组(P<0.05)。结论 中老年骨质疏松性单阶段胸腰椎压缩性骨折患者行经皮椎弓根钉内固定联合椎体成形术治疗,创伤小、术后疼痛轻,且有助于改善患者功能障碍与伤椎高度,值得临床推广应用。Objective To investigate the clinical effects of percutaneous pedicle screw internal fixation combined with vertebroplasty in the treatment of single segment thoracolumbar vertebral compression fractures with osteoporosis in middle-aged and elderly patients. Methods Clinical data of 60 middle-aged and elderly patients with single segment osteoporotic thoracolumbar compression fractures admitted to our hospital from March2019 to April 2020 were retrospectively analyzed, grouping according to different treatment methods, the patients treated with traditional open pedicle screw reduction and internal fixation were selected as the control group, and the patients treated with percutaneous pedicle screw internal fixation combined with vertebroplasty were selected as the observation group, with 30 cases in each group. The clinical indicators, pain degree, dysfunction,anterior height of injured vertebrae and kyphosis Cobb Angle before and after treatment were compared between the two groups. Results The operation time and incision length of the observation group were shorter than those of the control group, and the intraoperative blood loss was less than that of the control group(P<0.05). After treatment, visual analogue scale(VAS) scores, lumbar Oswestry dysfunction index(ODI)scores and kyphosis Cobb angle in both groups were lower than before treatment, and the observation group was lower than those of control group(P<0.05). After treatment, the anterior height of the injured vertebrae in both groups was higher than before, and the height of the anterior height of the injured vertebrae in the observation group was higher than that in the control group(P<0.05). Conclusion Percutaneous pedicle screw internal fixation combined with vertebroplasty in the treatment of middle-aged and elderly patients with single segment osteoporotic thoracolumbar compression fractures have less trauma and less postoperative pain, and can improve the dysfunction and height of injured vertebrae, which is worthy of clinical application.
