摘  要:目的 探讨短节段椎弓根钉固定手术治疗脊柱骨折的临床疗效和对脊柱功能的影响。方法 选取2017年11月至2020年7月于本院就诊的138例脊柱骨折患者作为研究对象,随机分为观察组和对照组,每组69例。对照组采用长节段椎弓根钉后路节内固定手术治疗,观察组采用短节段椎弓根钉固定手术治疗,比较两组治疗前后伤椎前缘高度比值和Cobb角及神经功能。结果 治疗前,两组伤椎前缘高度比值及Cobb角比较差异无统计学意义;治疗后,观察组伤椎前缘高度比值高于对照组,Cobb角变化低于对照组(P<0.05)。治疗前,两组神经功能比较差异无统计学意义;治疗后,观察组E级神经功能占比显著高于对照组,A级、B级均低于对照组(P<0.05)。结论 采用短节段椎弓根钉固定手术治疗脊柱骨折患者效果显著,可提高患者的伤椎前缘高度比值,降低侧弯角度,可有效改善神经功能损伤程度,提高手术安全性,值得临床推广应用。Objective To investigate the clinical effect of short-segment pedicle screw fixation in the treatment of spinal fractures and its influence on spinal function. Methods A total of 138 patients with spinal fractures who visited our hospital from November 2017 to July 2020 were selected as the research subjects, and they were randomly divided into observation group and control group, with 69 cases in each group. The control group was treated with long-segment pedicle screw fixation, while the observation group was treated with short-segment pedicle screw fixation, the ratio of the anterior edge height of the injured vertebrae, and the Cobb angle and neural function were compared between the two groups before and after treatment. Results Before treatment, there was no significant difference in the ratio of anterior edge height of injured vertebrae and Cobb angle between the two groups;after treatment, the ratio of anterior edge height of injured vertebrae in the observation group was higher than that in the control group, and the Cobb angle was lower than that in the control group(P<0.05). Before treatment, there was no significant difference in neural function between the two groups;after treatment, the proportion of Grade E nerve function in the observation group was significantly higher than that in the control group, and the proportion of grade A and B nerve function were lower than that in the control group(P<0.05). Conclusion The use of short-segment pedicle screw fixation in the treatment of patients with spinal fractures has a significant effect, which can increase the ratio of the anterior edge of the injured vertebrae, reduce the scoliosis angle, and improve the degree of neurological damage with high safety, it is worthy of clinical application.
