摘  要:目的探讨高频震荡通气联合牛肺表面活性物质在新生儿呼吸衰竭中的疗效及对炎症因子的影响。方法选取2018年1月至2020年6月因呼吸衰竭于本院接受治疗的46例新生儿作为研究对象,随机分为研究组(n=24)与对照组(n=22)。对照组采用高频振荡通气治疗,研究组采用高频振荡通气联合牛肺表面活性物质治疗,比较两组治疗前后血氧饱和度及血清炎症因子水平。结果治疗后,研究组血氧饱和度(SaO_(2))及动脉血氧分压(PaO_(2))均高于对照组,动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05),两组MAP指标比较差异无统计学意义;治疗后,研究组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及C反应蛋白(CRP)水平均低于对照组(P<0.05)。结论采用高频振荡通气联合牛肺表面活性物质治疗新生儿呼吸衰竭效果显著,能改善患儿血气分析指标,降低炎症因子水平,促进病情转归,值得临床推广应用。Objective To investigate the efficacy of high-frequency oscillatory ventilation combined with bovine pulmonary surfactant(Colisu)in neonatal respiratory failure and its effect on inflammatory factors.Methods A total of 46 neonates who were treated in our hospital for respiratory failure from January 2018 to June 2020 were selected as the research subjects,and they were randomly divided into the study group(n=24)and the control group(n=22).The control group was treated with high-frequency oscillatory ventilation,and the study group was treated with high-frequency oscillatory ventilation combined with bovine pulmonary surfactant.The blood oxygen saturation and serum inflammatory factor levels were compared between the two groups before and after treatment.Results After treatment,the indexes of oxygen saturation(SaO_(2))and partial pressure of oxygen(PaO_(2))in the study group were higher than those in the control group,and arterial blood carbon dioxide partial pressure(PaCO_(2))was lower than that in the control group(P<0.05).There was no significant difference in the mean arterial pressure(MAP)indexes between the two groups.After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in the study group were lower than those in the control group(P<0.05).Conclusion The use of high-frequency oscillatory ventilation combined with bovine pulmonary surfactant in the treatment of neonatal respiratory failure has significant effect,can improve the blood gas analysis indicators,reduce the level of inflammatory factors,and promote the prognosis of the disease,which is worthy of clinical application.
