摘  要:目的 探讨肌腱转位术治疗桡神经损伤晚期功能障碍的临床疗效。方法 选取2019年11月至2021年2月沈阳医学院附属中心医院收治的82例桡神经损伤晚期功能障碍患者作为研究对象,按照随机数字表法分为观察组和对照组,每组41例。对照组采用常规方法治疗,观察组采用肌腱转位术治疗,比较两组治疗前后桡神经支配指总伸肌的肌电图水平及治疗后关节功能恢复情况。结果 治疗前,两组桡神经运动神经传导速度、潜伏期及波幅变化比较差异无统计学意义;治疗后,观察组运动神经传导速度快于对照组,波幅高于对照组,潜伏期短于对照组(P<0.05)。治疗后,观察组关节功能恢复度为97.56%,高于对照组的80.49%(P<0.05)。结论 采用肌腱转位手术进行治疗桡神经损伤晚期功能障碍患者,可有效提高患者神经兴奋度,促进功能恢复,有效恢复腕关节功能,提高临床疗效,扩大关节活动范围。Objective To explore the clinical effect of tendon transposition in the treatment of late dysfunction of radial nerve injury. Methods 82patients with advanced dysfunction of radial nerve injury who were admitted to the Central Hospital of Shenyang Medical College from November2019 to February 2021 were selected as the research subjects, and they were divided into observation group and control group according to the random number table method, with 41 cases in each group. The control group were treated with conventional methods, and the observation group were treated with tendon transposition, the EMG levels of radial nerve innervated extensor digitorum common before and after treatment and the recovery of joint function after treatment were compared between the two groups. Results Before treatment, there was no significant difference in motor nerve conduction velocity of radial nerve, and the change of latency and amplitude between the two groups;after treatment, the motor nerve conduction velocity in the observation group was faster than that in the control group, the amplitude was higher than that in the control group, and the latency was shorter than that in the control group(P<0.05). After treatment, the total recovery degree of joint function in the observation group was97.56%, which was higher than 80.49% in the control group(P<0.05). Conclusion The treatment of patients with advanced dysfunction of radial nerve injury can effectively improve nerve excitability, promote functional recovery, effectively restore wrist joint function, improve clinical efficacy,and expand joint range of motion.
