摘 要:目的 探讨血栓弹力图(TEG)应用于花生四烯酸(AA)途径和二磷酸腺苷(ADP)途径血小板抑制率中的检测效果。方法 回顾性分析2020年4月至2021年3月于萍乡市人民医院行TEG检测ADP途径、AA途径血小板抑制率的126例患者的临床资料,根据临床服用抗血小板药物类型分为单纯A组(服用阿司匹林),单纯B组(服用氯吡格雷),联合C组(阿司匹林+氯吡格雷)和联合D组(阿司匹林+替格瑞洛)。比较不同途径患者的凝血功能[D-二聚体(D-D)、纤维蛋白原(FIB)、凝血酶原时间(PT)]及血脂水平[三酰甘油(TG)、胆固醇(CHO)],比较不同用药患者ADP、AA途径血小板抑制率疗效。结果 ADP途径检测TG结果明显低于AA途径,差异有统计学意义(P<0.05)。联合D组ADP途径血小板抑制率明显高于单纯A组、单纯B组、联合C组,差异有统计学意义(P<0.05)。单纯A组AA途径血小板抑制率明显高于联合D组、联合C组、单纯B组,差异有统计学意义(P<0.05)。结论 ADP途径血小板抑制率检测需评估TG指标,在ADP途径血小板抑制率疗效中阿司匹林联合应用替格瑞洛疗效显著,在AA途径血小板抑制率疗效上阿司匹林应用疗效显著。Objective To explore the effect of thromboelastography(TEG) in the detection of platelet inhibition rate in arachidonic acid(AA)pathway and adenosine diphosphate(ADP) pathway. Methods The clinical data of 126 patients who underwent TEG to detect the platelet inhibition rate of ADP pathway and AA pathway in Pingxiang City People’ s Hospital from April 2020 to March 2021 were retrospectively analyzed, they were divided into simple group A(aspirin), simple group B(clopidogrel), combined group C(aspirin+clopidogrel) and combined group D(aspirin + ticagrelor) according to the type of antiplatelet drugs taken clinically. The coagulation function(D-dimer [D-D], fibrinogen [FIB], prothrombin time[PT]) and blood lipid levels [triglyceride(TG), cholesterol(CHO)] were compared of patients with different pathways, the efficacy of platelet inhibition rate of ADP and AA pathways were compared in patients with different medication. Results The results of TG detected by ADP pathway were significantly lower than those detected by AA pathway, and the difference was statistically significant(P<0.05). The platelet inhibition rate of ADP pathway in combined group D was significantly higher than that in simple group A, simple group B, and combined group C, and the difference was statistically significant(P<0.05). The platelet inhibition rate of AA pathway in simple group A was significantly higher than that in combined group D, combined group C, and simple group B, and the difference was statistically significant(P<0.05). Conclusion The detection of ADP pathway platelet inhibition rate needs to evaluate TG indicators, in the efficacy of ADP pathway platelet inhibition rate, the application of aspirin combined ticagrelor has significant effect, and in the AA pathway platelet inhibition rate, aspirin has significant effect.
