摘 要:目的 探究成人股骨头坏死早期诊断中螺旋CT与高场MRI的应用价值。方法 回顾性分析2020年6月至2021年2月泰安市中医二院收治的124例成人股骨头坏死患者的高场MRI、螺旋CT影像学检查资料,分析比较两种诊断方式的诊断准确率、影像学特征。结果 124例成人股骨头坏死患者共有194髋确诊,MRI共确诊194髋,确诊率为100.00%,Ⅰ期、Ⅱ期、Ⅲ期分别有22髋、76髋、96髋,螺旋CT共确诊172髋,确诊率为88.66%,Ⅰ期、Ⅱ期、Ⅲ期分别有0髋、72髋、100髋,MRI的Ⅰ期股骨头病变诊断准确率明显高于螺旋CT(P<0.05);MRI检查显示,136髋单一囊变(70.10%),142髋骨髓水肿(73.20%),128髋出现线样征(65.98%),22髋骨小梁结构模糊(11.34%),经螺旋CT检查显示,136髋单一囊变(70.10%),116例出现斑片状高密度影,且骨小梁结构十分模糊(59.79%)。结论 相较于螺旋CT,MRI在成人股骨头坏死早期诊断中的应用价值更高。Objective To explore the application value of spiral CT and high-field MRI in the early diagnosis of adult femoral head necrosis.Methods The high-field MRI and spiral CT imaging data of 124 adult patients with femoral head necrosis admitted to Fushun Central Hospital in Liaoning Province from July 2015 to December 2019 were retrospectively analyzed, and the two diagnostic methods were diagnosed. The accuracy rate and imaging characteristics were compared and analyzed. Results In 124 adult patients with femoral head necrosis, a total of 194 hips were diagnosed. MRI diagnosed a total of 194 hips, the diagnosis rate was 100.00%. There were 22 hips, 76 hips, and 96 hips in stage Ⅰ, stage Ⅱ, and stageⅢ, respectively, and a total of 172 hips were diagnosed by spiral CT, the diagnosis rate was 88.66%. There were 0 hips, 72 hips, and 100 hips in stageⅠ, stage Ⅱ, and stage Ⅲ, respectively. The diagnostic accuracy of stage Ⅰ femoral head lesions by MRI was significantly higher than that of spiral CT(P<0.05);MRI examination showed, 136 hips have single cyst degeneration(70.10%), 142 hips have bone marrow edema(73.20%), 128 hips have line-like signs(65.98%), 22 hips have blurred trabecular structure(11.34%), as showed by spiral CT examination, 136 single cyst of the hip(70.10%), 116 cases showed patchy high-density shadows, and the trabecular bone structure was very blurred(59.79%). Conclusion Compared with spiral CT, MRI has higher application value in the early diagnosis of adult femoral head necrosis.
