摘 要:目的 分析动态心电图下ST-T改变联合QTc间期预测冠心病患者心肌缺血发作的临床价值。方法 选取2018年1月至2020年5月于本院接受诊治的80例冠心病患者作为研究对象,根据冠状动脉造影检测结果将心肌缺血未发作患者纳入对照组,心肌缺血发作患者纳入观察组,每组40例。两组均进行动态心电图检测,预测分析其动态心电图下QTc间期、ST-T段改变以及ST-T改变与QTc间期联合预测冠心病患者心肌缺血发作结果。结果 两组心率、ST段比较差异无统计学意义;观察组QTc间期、ST-T段改变占比均高于对照组,差异有统计学意义(P<0.05)。ST-T段改变、QTc间期两种方法预测灵敏度、特异度、准确度比较差异无统计学意义;ST-T段改变联合QTc间期预测灵敏度、特异度、准确度均高于ST-T段改变、QTc间期,差异有统计学意义(P<0.05)。结论 动态心电图下ST-T改变联合QTc间期对冠心病患者心肌缺血发作的预测灵敏度、特异度、准确度较高,具有一定的临床诊断价值,但仍需结合其他检测方法共同诊断,提高确诊准确率。Objective To analyze the clinical value of ST-T changes combined with QTc interval under dynamic electrocardiogram in predicting myocardial ischemic attack in patients with coronary heart disease. Methods A total of 80 patients with coronary heart disease who received diagnosis and treatment in our hospital from January 2018 to May 2020 were selected as the research objects. According to the results of coronary angiography, the patients without myocardial ischemia were included in the control group, and the patients with myocardial ischemia were included in the observation group, with 40 cases in each group. The control group was the patients without myocardial ischemia, and the observation group was the patients with myocardial ischemia. The two groups were detected by dynamic electrocardiogram, and the QTc interval and ST-T segment changes under the dynamic electrocardiogram were predicted and analyzed, and the combination of ST-T changes and QTc interval predicted the outcome of myocardial ischemic attack in patients with coronary heart disease. Results There was no significant difference in heart rate and ST segment between the two groups;the proportion of changes in QTc interval and ST-T segment in the observation group were higher than those in the control group, and the difference was statistically significant(P<0.05). There was no significant difference in the sensitivity, specificity and accuracy of STT segment change and QTc interval prediction between the two methods;the sensitivity, specificity and accuracy of ST-T segment change combined with QTc interval prediction were higher than those of ST-T segment change and QTc interval, and the difference was statistically significant(P<0.05). Conclusion ST-T changes combined with QTc interval under dynamic electrocardiography have high sensitivity, specificity and accuracy in predicting myocardial ischemic attack in patients with coronary heart disease, and have certain clinical diagnostic value, but it still needs to be combined with other detection method
