小剂量甲氨蝶呤和腹腔镜保守手术治疗异位妊娠的临床疗效

摘　　要：目的 探讨小剂量甲氨蝶呤与腹腔镜保守手术治疗异位妊娠(EP)的临床疗效。方法 选取2017年6月至2018年6月本院收治的88例EP患者作为研究对象,采用随机数字表法分为两组,各44例。对照组采取腹腔镜保守手术治疗,实验组采取小剂量甲氨蝶呤与腹腔镜保守手术治疗,比较两组输卵管再通率、持续性EP发生率、正常妊娠率、EP再发生率及治疗后基本情况。结果 实验组输卵管再通率、正常妊娠率均高于对照组,持续性EP发生率、EP再发生率均低于对照组(P<0.05);实验组术中失血量少于对照组,血清β-人绒毛促性腺激素(β-hCG)恢复至正常时间及住院时间均短于对照组(P<0.05)。结论 腹腔镜保守手术与小剂量甲氨蝶呤治疗EP患者效果显著,可促进患者输卵管再通,提高正常妊娠率,降低EP再发生率,减少术中失血量,缩短患者血清β-HCG恢复至正常时间,促进患者康复,值得临床推广应用。Objective To investigate the effect of low-dose methotrexate and laparoscopic conservative surgery in the treatment of ectopic pregnancy(EP). Methods A total of 88 EP patients admitted to our hospital from June 2017 to June 2018 were selected as the research subjects, and they were divided into two groups according to random number table method, with 44 cases in each group. The control group was treated with laparoscopic conservative surgery, while the experimental group was treated with low-dose methotrexate and laparoscopic conservative surgery. The rate of tubal recanalization, the incidence of persistent EP, the normal pregnancy rate, the recurrence rate of EP and the basic situation after treatment were compared between the two groups. Results The tubal recanalization rate and normal pregnancy rate in the experimental group were higher than those in the control group, and the incidence of persistent EP and EP recurrence were lower than those in the control group(P<0.05);the blood loss in the experimental group was less than that in the control group, the time for β-human chorionic gonadotropin(β-hCG) to return to normal and the length of hospital stay were shorter than those in the control group(P<0.05). Conclusion Laparoscopic conservative surgery and low-dose methotrexate have significant effects in the treatment of EP patients, which can promote the recanalization of fallopian tubes, increase the normal pregnancy rate,reduce the recurrence rate of EP, reduce the amount of blood loss during surgery, and shorten the recovery of serum β-HCG to normal, it is worthy of clinical promotion and application.
