朱章志治疗成人隐匿性自身免疫糖尿病用药规律分析

摘　　要：【目的】探索朱章志教授治疗成人隐匿性自身免疫糖尿病(LADA)的用药规律。【方法】收集整理朱章志教授治疗LADA住院患者的处方,应用古今医案云平台,采用软件集成的频次分析、聚类分析、复杂网络分析等方法,探索朱章志教授治疗LADA的用药规律。【结果】筛选后共得到治疗少阴阳虚寒湿证LADA患者的处方36首,用药69味;治疗少阳失枢致胆火内郁证LADA患者的处方39首,用药75味;所用中药的性味以甘温为主,主要归脾、肺经。通过聚类分析和复杂网络分析得到治疗少阴阳虚寒湿证LADA的衍化方的药物组成为附子、红参、干姜、白术、黄芪、炙甘草、茯苓、白芍、生姜、吴茱萸、山茱萸、麻黄、桂枝、泽泻、细辛、肉桂、砂仁,治疗少阳失枢致胆火内郁证LADA的衍化方的药物组成为柴胡、黄芩、法半夏、干姜、茯苓、炙甘草、桂枝、白芍、生姜、大枣、红参、泽泻、天花粉、白术。【结论】朱章志教授治疗LADA辨治以少阴阳虚寒湿证和少阳失枢致胆火内郁证为主,多采用温阳散寒、和解少阳、调中补虚等药物,其组方思路可为临床诊疗LADA提供参考。Objective To investigate Professor ZHU Zhang-Zhi’s medication rules for the treatment of latent autoimmune diabetes in adults(LADA).Methods The prescriptions for LADA inpatients prescribed by Professor ZHU Zhang-Zhi’s were collected and sorted out,and then the medication rules for the treatment of LADA by Professor ZHU Zhang-Zhi were mined out with the software-integrated frequency analysis,cluster analysis and complex network analysis based on the Ancient and Modern Medical Record Cloud Platform.Results A total of 36 prescriptions with 69 herbs were obtained for the treatment of LADA patients due to Shaoyin yang-deficiency and cold-damp syndrome,and 39 prescriptions with 75 herbs were obtained for the treatment of patients with the internal accumulation of gallbladder fire syndrome due to Shaoyang dysfunction.The herbs for LADA were predominated by the sweet-warm nature and with the meridian tropism of spleen and lung.The results of cluster analysis and complex network analysis showed that formula for the treatment of Shaoyin yang-deficiency and colddamp syndrome derived from the core herbs was composed of Radix Aconiti Lateralis Preparata,Radix Ginseng Rubra,Rhizoma Zingiberis,Rhizoma Atractylodis Macrocephalae,Radix Astragali,Radix Glycyrrhizae Preparata,Poria,Radix Paeoniae Alba,Rhizoma Zingiberis Recens,Fructus Evodiae,Fructus Corni,Herba Ephedrae,Ramulus Cinnamomi,Rhizoma Alismatis,Herba Asari,Cortex Cinnamomi and Fructus Amomi.And the formula for the treatment of the internal accumulation of gallbladder fire syndrome due to Shaoyang dysfunction was composed of Radix Bupleuri,Radix Scutellariae,Rhizoma Pinelliae Preparata,Rhizoma Zingiberis,Poria,Radix Glycyrrhizae Preparata,Ramulus Cinnamomi,Radix Paeoniae Alba,Rhizoma Zingiberis Recens,Fructus Jujubae,Radix Ginseng Rubra,Rhizoma Alismatis,Radix Trichosanthis,and Rhizoma Atractylodis Macrocephalae.Conclusion For the treatment of LADA,Professor ZHU Zhang-Zhi usually differentiates LADA into Shaoyin yang-deficiency and cold-damp syndrome and internal acc
