益气养阴通淋方治疗气阴虚夹湿热型复发性尿路感染的临床观察

摘　　要：【目的】观察益气养阴通淋方治疗气阴虚夹湿热型复发性尿路感染(RUTI)的临床疗效。【方法】将68例气阴虚夹湿热型RUTI女性患者随机分为对照组和治疗组,每组各34例。对照组给予口服左氧氟沙星片抗感染治疗,治疗组在对照组的基础上加用中药益气养阴通淋方加减治疗。7 d为1个疗程,连续治疗2个疗程,并于治疗结束后的第6周随访。观察2组患者治疗前后中医证候积分的变化情况,比较2组患者的临床疗效、复发率和不良反应发生情况。【结果】(1)研究过程中,共有5例患者因各种原因未能按照课题设计进行治疗和随访而脱落,最终共有63例患者纳入统计分析,其中,对照组31例,治疗组32例。(2)治疗2周后,治疗组的总有效率为90.63%(29/32),对照组为61.29%(19/31),组间比较,治疗组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,2组患者的中医证候积分均较治疗前明显降低(P<0.05),且治疗组的降低作用明显优于对照组,组间治疗后及差值比较,差异均有统计学意义(P<0.05)。(4)于治疗结束后的第6周随访,治疗组的复发率为18.75%(6/32),对照组为41.94%(13/31),组间比较,治疗组的复发率明显低于对照组,差异有统计学意义(P<0.05)。(5)治疗组在服药第2周出现1例呕吐患者;对照组在服药第1周出现3例皮疹、瘙痒患者和2例恶心、呕吐患者,在服药第2周出现2例腹泻患者;其余未出现不良反应。【结论】益气养阴通淋方治疗气阴虚夹湿热型RUTI患者疗效确切,可有效改善临床症状,降低复发率,且无明显不良反应。Objective To observe the clinical efficacy of Yiqi Yangyin Tonglin Formula for the treatment of recurrent urinary tract infection(RUTI)of qi and yin deficiency complicated with damp-heat type.Methods Sixtyeight female RUTI patients with qi and yin deficiency complicated with damp-heat type were randomly divided into the control group and the treatment group,34 patients in each group.The control group was given oral use of antiinfection treatment with levofloxacin tablets,while the treatment group was treated with the Chinese herbal medicine Yiqi Yangyin Tonglin Formula orally based on the treatment for the control group.Seven days constituted a course of treatment,and the treatment for the two groups lasted for 2 consecutive courses.At the 6th week after the completion of treatment,the patients were followed up.The changes of the traditional Chinese medicine(TCM)syndrome scores were observed before and after treatment,and the clinical efficacy,recurrence rate and adverse reactions of the two groups were also compared.Results(1)During the trial,a total of 5 patients fell off due to the failure of being treated and followed up according to the subject design for various reasons,and eventually a total of 63 patients were included in the statistical analysis,of which 31 were in the control group and 32 were in the treatment group.(2)After 2 weeks of treatment,the total effective rate of the treatment group was 90.63%(29/32)and that of the control group was 61.29%(19/31).The intergroup comparison showed that the efficacy of the treatment group was significantly superior to that of the control group,and the difference was statistically significant(P<0.05).(3)After treatment,the TCM syndrome scores of both groups were significantly decreased compared with those before treatment(P<0.05).The effect on lowering the scores in the treatment group was significantly superior to that in the control group,and statistically significant difference was shown in the posttreatment scores between the two groups and in the pre-and post
