百合利咽茶治疗肺肾阴虚型慢喉痹的疗效及其网络药理学机制探讨

摘　　要：【目的】基于网络药理学,探讨百合利咽茶治疗肺肾阴虚型慢喉痹(慢性咽炎)的疗效机制。【方法】将85例肺肾阴虚型慢喉痹患者随机分成2组,其中观察组45例,对照组40例。根据中医辨证论治的理论,对照组在第1周至第4周给予养阴清肺汤为期4周的治疗,而从第5周至第8周不再进行任何药物治疗和干预;观察组在第1周至第4周给予养阴清肺汤联合百合利咽茶为期4周的治疗,从第5周开始至第8周将不使用养阴清肺汤治疗,但继续使用百合利咽茶作为日常茶饮。2组均在治疗4周后和治疗8周后进行疗效评价。同时,通过比较毒理基因组学数据库(CTD)及GeneCards数据库下载与慢喉痹相关的基因列表;应用BATMAN-TCM数据库对百合利咽茶的活性成分及作用靶点进行预测分析;进而获取百合利咽茶与慢喉痹的交集共同靶点,并使用String构建共同靶点网络;对交集共同靶点基因在GO和KEGG数据库进行富集分析,以预测百合利咽茶辅助治疗慢喉痹的可能作用机制。【结果】(1)临床疗效方面,治疗4周后,观察组的总有效率为82.2%(37/45),高于对照组的65.0%(26/40),但差异无统计学意义(P>0.05)。治疗8周后,观察组的总有效率为91.1%(41/45),明显高于对照组的70.0%(28/40),差异有统计学意义(P<0.05)。(2)网络药理学方面,找到与慢喉痹的相关靶点有1008个,在设置相关分数>90分及P值<0.05的参数后,筛选出153个与百合利咽茶相关的靶点,两者交集后有21个相关靶点。在KEGG和GO富集结果中,百合利咽茶的显著治疗靶点在“细胞修饰的氨基代谢通路”和“谷胱甘肽代谢”。【结论】百合利咽茶在肺肾阴虚型慢喉痹的辅助治疗及治疗后的预防保健中可发挥显著的疗效。基于网络药理学,其对慢喉痹疾病相关的多个靶点和多条信号通路的调控是提高慢喉痹治疗有效率的重要作用机制。Objective To investigate the therapeutic mechanism of Baihe Liyan Tea for the treatment of chronic sore throat,i.e.,chronic pharyngitis,with lung and kidney yin deficiency type based on network pharmacology.Methods Eighty-five chronic pharyngitis patients with lung and kidney yin deficiency type were randomly divided into two groups,45 cases in the observation group and 40 cases in the control group.According to the traditional Chinese Medicine(TCM)syndrome differentiation,the control group was treated with Yangyin Qingfei Decoction with the actions of nourishing yin and clearing lung for 4 weeks from week one to week 4,and then had no further medication or intervention from week 5 to week 8.The observation group was treated with Yangyin Qingfei Decoction combined with Baihe Liyan Tea for 4 weeks from week one to week 4,and then had no treatment with Yangyin Qingfei Decoction but Baihe Liyan Tea was still used as the daily tea from week 5 to week 8.The efficacy of the two groups was evaluated after 4 weeks and 8 weeks of treatment.Meanwhile,the gene lists associated with hronic laryngitis were downloaded from Comparative Toxicogenomics Database(CTD)and GeneCards database.The BATMAN-TCM database was used to predict the active ingredients and action targets of Baihe Liyan Tea,the intersecting targets of Baihe Liyan Tea for chronic laryngitis were obtained for the construction of common target network with String database.And then the enrichment analysis was carried out on GO and KEGG databases to predict the possible therapeutic mechanism of Baihe Liyan Tea for the treatment of chronic pharyngitis.Results(1)After 4 weeks of treatment,the total effective rate of the observation group was 82.2%(37/45)and was higher than that of the control group[65.0%(26/40)],but the difference was not statistically significant between the two groups(P>0.05).After 8 weeks of treatment,the total effective rate of the observation group was 91.1%(41/45),which was significantly higher than that of the control group[70.0%(28/40)],and the
