面部胶样粟丘疹一例

摘　　要：47岁女性患者,面部黄色斑丘疹、斑块3年余,加重2年。查体可见面部及鼻尖多发蜡黄色斑丘疹、丘疹及斑块,相互融合,表面光滑,无破溃、糜烂。皮肤镜检查可见橙黄色背景下毛细血管扩张及粟粒样囊肿。皮损组织病理见真皮中上层大量团块状均质红染物质;PAS、甲紫及刚果红染色阳性:免疫组化染色示κ蛋白阳性,λ蛋白部分阳性,CD79α散在阳性。诊断:胶样粟丘疹。A 47 years old female presented with yellow maculopapules and plaques on face for more than 3 years and aggravated for 2 years.Mutiple waxy yellow maculopapules,paqules and plaques could be seen on the face and tip of nose.The surface of lesions was smooth without ulceration and erosion.Under the dermoscope,capillary dilation and miliary cysts could be seen under the orange background.Histopathological examination showed a large amount of homogeneous red staining in the middle and upper dermis.The lesions were positive for PAS staining,methyl violet staining and congo red staining,and for immunohistochemical staining of proteinκ.The lesions were partial positive for immunohistochemical staining of proteinλ,and scattered positive for immunohistochemical staining of CD79α.This case was diagnosis as colloid milium.
