三尖瓣环收缩期位移评价正常中、晚孕期胎儿右心室功能的定量研究

摘　　要：目的分析胎儿三尖瓣环收缩期位移(fetal-tricuspid annular plane systolic excursion,f-TAPSE)作为评估胎儿右心室收缩及舒张功能指标的应用价值以及随孕周的变化规律,并建立正常胎儿中孕组、晚孕组f-TAPSE的测量值参考范围。方法选取135例正常中、晚孕期胎儿,应用M型超声于三尖瓣环水平测量f-TAPSE,分析其随孕周的变化规律,以及其与右心室功能常用测量指标的相关性。不同孕周组间f-TAPSE、心肌组织多普勒舒张早期速度(Ea)、舒张晚期速度(Aa)、心肌组织收缩期峰值血流速度(Sa)、右心室每搏输出量(RSV)及右心室的射血分数(RVEF)等右室功能常用测量指标的比较采用单因素方差分析,各因素间的相关性分析采用Pearson相关分析。结果正常中、晚孕胎儿不同孕周组间f-TAPSE组间比较差异具有统计学意义,其与Ea、Aa、Sa、RSV等右心室功能常用指标测值呈显著正相关(P<0.05),但与RVEF组间比较差异不具有统计学意义(P>0.05)。结论f-TAPSE可以作为定量反映右心室收缩及舒张功能的指标。中孕组f-TAPSE参考值范围:3.21~6.12,晚孕组f-TAPSE参考值范围:4.74~9.68。Objective To analyze f-TAPSE as an index for assessing fetal right ventricular systolic and diastolic function and the variation pattern with gestational weeks.Simultaneously,establish the f-TAPSE measurement reference ranges of second-and third-trimester of pregnancy.Methods The f-TAPSE was measured at the level of the tricuspid annulus using M-mode ultrasound in 135 normal second-and third-trimester of pregnancy fetuses,and its variation with gestational week and its correlation with common measures of right ventricular function were analyzed.One-way ANOVA was used to compare f-TAPSE,Ea,Aa,Sa,RSV,and RVEF among different gestational week groups,and Pearson correlation analysis was used to analyze the correlation between the factors.Results The differences between f-TAPSE groups were statistically significant in normal second-and third-trimester of pregnancy fetuses compared between different gestational weeks,and they were significantly and positively correlated with the measured values of Ea,Aa,Sa,RSV,and other common indexes of right ventricular function(P<0.05),but not with the RVEF group(P>0.05).Conclusion f-TAPSE can be used as a quantitative index of right ventricular systolic and diastolic function.f-TAPSE reference ranges are 3.21-6.21 in second-trimester of pregnancy,and 4.74-9.68 in third-trimester of pregnancy.
