摘　　要：目的探讨NLRP3炎症小体与心房颤动(AF)及CHADS-VASc评分的关系,明确NLRP3炎症小体在AF中的可能作用。方法连续入选住院治疗的161例AF患者为研究对象,其中阵发性AF组56例、持续性AF组60例、长程持续性AF组45例。并选择50例健康体检者纳入对照组。利用梯度离心法分离纯化外周血单核细胞(PBMC),采用实时定量PCR和Western blot检测NLRP3的mRNA和蛋白表达。结果各AF组患者年龄均高于对照组(P<0.01)。与对照组相比,各组AF患者PBMC中NLRP3蛋白和mRNA水平均升高(P<0.01)。长程持续性AF组NLRP3蛋白和mRNA水平高于阵发性AF组和持续性AF组(P<0.05)。AF患者血浆IL-1β水平高于对照组(P<0.01)。而长程持续性AF患者血浆IL-1β水平高于阵发性AF组和持续性AF组(P<0.05)。AF患者血浆IL-1β水平随着CHA2DS2-VASc评分的升高而升高(P<0.05)。结论NLRP3炎症小体与AF和CHA_(2)DS_(2)-VASc评分密切相关,NLRP3炎症小体可能成为AF和AF相关卒中的潜在防治靶点。Objective To explore the correlation between NLRP3 inflammasome and atrial fibrillation(AF)and CHADS-VASc score,and to clarify the possible role of NLRP3 inflammasome in AF.Methods Peripheral blood mononuclear cells(PBMC)were isolated and purified by gradient centrifugation.The mRNA and protein expression levels of NLRP3 in PBMC were detected by real-time quantitative PCR and Western Blot,respectively.Results The age of AF patients was higher than that of control group(P<0.01).Compared with the control group,the NLRP3 protein and mRNA levels in PBMC of AF patients were considerably increased(P<0.01).The protein and mRNA expression levels of NLRP3 in patients with long-term persistent AF were higher than those in patients with paroxysmal AF and persistent AF(P<0.05).Compared with the control group,the plasma IL-1βlevel of AF patients was significantly increased(P<0.01).The plasma IL-1βlevel of patients with long-term persistent AF was higher than that of patients with paroxysmal AF and persistent AF(P<0.05).Moreover,plasma IL-1βlevel of AF patients increased with CHADS-VASc score(P<0.05).Conclusion NLRP3 inflammasome is closely related to AF and CHA_(2)DS_(2)-VASc score.NLRP3 inflammasome may be a potential target for the prevention and treatment of AF and AF-related stroke.
