安宁家庭会议中家属互动方式多模态研究——“病房婚礼”案例分析

摘　　要：本研究以一场安宁疗护家庭会议的全程录像为语料,采用多模态话语分析的方法,考察家庭会议中主要知情/决策家属与医生及团队互动中的功能、多模态配置和决策结果。研究显示,患者儿子小马通过使用多模态资源,实现了主动知情、开放表达、积极创意并行动的功能。不同的“高层活动”采用了不同的模态配置:病情沟通以语言为主导,模态配置简单集中,小马在理性层面意识到病情危重的现实。情绪表达的模态配置丰富复杂,综合采用了语言和各种副语言、非语言资源,表达了假如不能实现父亲心愿的遗憾。决策活动是情感驱动的行动协商,以语言的主导和模态的多元为特征。在家属和安宁团队的共同构建下,“病房婚礼”的创意得以具体化,并最终实现,生死两安。家属的主动知情、开放表达、积极创意并行动,可视为死亡话语素养的一部分,具有实践启示。Focusing on the decision-making family member Xiao Ma in a palliative care family meeting,this case study examined the communicative functions and modal configurations in his interaction primarily with the palliative care team.The data consisted of the complete video recording of a palliative care family meeting,and the analytical framework adopted was that of Norris(2004,2019).Results showed that Xiao Ma employed different modal configurations in higher-level activities of information reception,emotion expression,and decision formulation.The solution of this meeting was a“wedding in the hospice ward”enabling Xiao Ma’s dying father to see his son get married.Xiao Ma’s active information reception,open emotion expression,creative decision formulation and negotiation might be part of the“death literacy”for family members of the dying,which calls for future research and practice particularly in the Chinese context.
