常态化疫情防控下可视化标准作业程序在医院保洁管理中的应用

摘　　要：目的 探讨常态化疫情防控下可视化标准作业程序(standard operation procedure,SOP)在医院保洁管理中的应用效果,为制定院内感染防控措施提供参考。方法 依据院内感染防控相关技术规范并结合河南省某三甲医院实际情况,制定保洁可视化SOP;对全院保洁人员进行可视化SOP宣教和培训后实施;利用同一考核标准,分别在可视化SOP实施前与实施后对全院从事保洁工作≥1年的199名保洁人员进行考核。结果 可视化SOP实施后与实施前相比,199名保洁人员在感染防控基本知识知晓率(100.00%vs 97.49%)、防护用品选用及穿脱合格率(71.86%vs 41.21%)、手卫生依从率(89.95%vs 80.90%)、手卫生正确率(82.12%vs 65.84%)、清洁工具分区使用正确率(85.43%vs 49.75%)、消毒液配制正确率(72.86%vs 45.23%)、荧光标记清除合格率(91.96%vs 77.89%)、物表微生物监测合格率(88.94%vs71.86%)、医疗废物收集正确率(78.89%vs 51.76%)和职业暴露处置正确率(86.43%vs 48.24%)均有提升,且除感染防控基本知识外,其他项目差异均有统计学意义(均P<0.05)。结论 实施可视化SOP,能够提高医院保洁人员的职业素养,有效减少医疗机构内交叉感染,保障医疗安全。Objective To explore the application effect of visual standard operation procedure(SOP) in the management of hospital cleaners under the normalization of COVID-19 prevention and control.Methods According to relevant requirements and realities in the hospital,we developed visual SOP related to cleaning.The visual SOP education and training for the whole cleaners of the hospital were carried out.Using the same assessment standard,we compared the management effect of hospital cleaners before and after the implementation of visual SOP.Results Compared with before the implementation of the Visual SOP,the awareness rate of basic knowledge of infection prevention and control(100.00% vs 97.49%),the qualified rate of selection and removal of protective equipment(71.86% vs 41.21%),the compliance rate of hand hygiene(89.95% vs 80.90%),correct rate of hand hygiene(82.12% vs 65.84%),the correct rate of partition use of cleaning tools(85.43% vs 49.75%),the correct rate of disinfection solution preparation(72.86% vs 45.23%),the clearance rate of fluorescent labels(91.96% vs 77.89%),the qualified rate of microbial monitoring on the surface(88.94% vs 71.86%),the correct rate of medical waste collection(78.89% vs 51.76%)and the correct rate of occupational exposure disposal(86.43% vs 48.24%) were increased among 199 cleaners.Except for the basic knowledge of infection prevention and control,the differences in the other items were statistically significant(all P<0.05).Conclusion The implementation of visual SOP can effectively improve the qualities and abilities of hospital cleaners,strictly prevent cross-infection in hospitals,and ensure medical safety.
