摘 要:目的 掌握漯河市医院消毒质量状况,为提高医院消毒工作质量和制定医院感染防控措施提供依据。方法 采取整群分层抽样方法,2020年抽取漯河市15家医院,按照《医院消毒卫生标准GB15982-2012》等要求,开展软式内镜、污水、医疗用水、环境空气、医护人员手卫生、医疗器械和物体表面等消毒质量监测。结果 2020年共监测样品3 887份,总合格率93.93%(3 651/3 887),其中三级医院、二级医院和乡镇卫生院合格率分别为99.00%(688/695)、95.73%(1 265/1 312)和90.80%(1 707/1 880),不同级别医院合格率差异有统计学意义(χ2=71.045,P<0.001);不同监测项目合格率分别为软式内镜100.00%(59/59),医院污水89.47%(51/57),医疗用水85.83%(430/501),环境空气91.89%(839/913),医护人员手卫生96.49%(605/627),医疗器械98.43%(250/254),重点科室物体表面96.00%(1 417/1 476)。不同级别医院医疗用水、环境空气、医护人员手卫生及重点科室物体表面合格率差异有统计学意义(P值均<0.05)。结论 漯河市医院消毒质量整体较好,但医疗用水、医院污水和环境空气等项目消毒质量欠佳,建议加强基层医疗机构消毒与感染防控工作,保障医患安全。Purpose To master the quality of hospital disinfection in Luohe,and provide evidence for improving the quality of hospital disinfection and formulating hospital infection prevention and control measures.Methods Stratified cluster sampling method was adopted to select 15 hospitals in Luohe city in 2020,and disinfection quality monitoring of soft endoscope,sewage,medical water,ambient air,hand hygiene of medical staff,medical instruments and object surfaces according to the requirements of GB15982-2012 Hygienic Standard for Hospital Disinfection was carried out.Results In 2020,a total of 3 887 monitored samples were collected,with an overall qualification rate of 93.93%(3 651/3 887).Of them,the qualification rate was 99.00%(688/695) in tertiary hospitals,95.73%(1 265/1 312) in secondary hospitals,and 90.80%(1 707/1 880) in township hospitals.There was significant difference in the qualification rates among different levels of hospitals(χ2=71.045,P<0.001).The qualification rates by different monitoring items were 100.00%(59/59) for soft endoscopy,89.47%(51/57) for hospital sewage,85.83%(430/501) for medical water,91.89%(839/913) for ambient air,96.49%(605/627) for medical staff hand hygiene,98.43%( 250/254)for medical equipment and 96.00%(1 417/1 476) for the object surface in key departments.The differences of qualification rates for medical water,ambient air,hand hygiene of medical staff and object surfaces in different levels of hospitals were statistically significant(all P<0.05).Conclusion The overall quality of hospital disinfection in Luohe is good,but some items such as medical water,hospital sewage and ambient air are weak links.It is suggested to strengthen the disinfection and infection prevention and control work in primary medical institutions to ensure the safety of doctors and patients.
