逐瘀豁痰汤联合针刀治疗缺血性中风后痉挛性偏瘫临床观察

摘　　要：目的观察逐瘀豁痰汤联合针刀治疗缺血性中风后痉挛性偏瘫的疗效。方法从缺血性中风后痉挛性偏瘫的患者中选取痰浊型患者96例,随机分为试验组(48例)及对照组(48例)。2组患者均进行基础康复及针刀治疗,试验组服用逐瘀豁痰汤,对照组服用半夏白术天麻汤,连续治疗4周。对比2组患者治疗前、治疗4周后的Barthel指数(Barthel index,BI)、Fugl-Meyer运动功能评分(Fugl-Meyer assessment,FMA)、改良Ashworth痉挛量表(Modified Ashworth scale,MAS)及临床痉挛指数评分(Clinic spasticity index,CSI)。结果治疗4周后,2组患者BI和FMA评分均高于治疗前(P<0.05),且试验组BI和FMA评分均高于对照组(P<0.05);治疗4周后,2组患者MAS、CSI评分均低于治疗前(P<0.05),且试验组MAS、CSI评分均低于对照组(P<0.05)。结论逐瘀豁痰汤联合针刀疗法可改善缺血性中风后痉挛性偏瘫患者的肌张力、缓解痉挛,提高患者的运动功能和生活质量,疗效优于半夏白术天麻汤联合针刀疗法,值得在临床治疗中推广应用。Objective To observe the efficacy of Zhuyu Huotan decoction combined with acupotomy in the treatment of spastic hemiplegia after ischemic stroke.Methods 96 patients with sputum turbidity were selected from patients with spastic hemiplegia after ischemic stroke,and they were randomly divided into an experimental group(48 cases)and a control group(48 cases).The two groups of patients received basic rehabilitation and acupotomy treatment.The experimental group took Zhuyu Huotan decoction,and the control group took Banxia Baizhu Tianma decoction for 4 consecutive weeks.The Barthel index(BI),Fugl-Meyer assessment(FMA),modified Ashworth scale(MAS)and clinic spasticity index(CSI)were compared between the two groups before treatment and after 4 weeks of treatment.Results After 4 weeks of treatment,the BI and MAS scores of the two groups were higher than before treatment(P<0.05),and the BI and MAS scores of the experimental group were higher than those of the control group(P<0.05).After 4 weeks of treatment,the MAS and CSI scores of the two groups were lower than before treatment(P<0.05),and the MAS and CSI scores of the experimental group were lower than those of the control group(P<0.05).Conclusion Zhuyu Huotan decoction combined with acupotomy therapy can improve muscle tension,relieve spasticity,and improve the motor function and quality of life of patients with spastic hemiplegia after ischemic stroke.The curative effect is better than that of Banxia Baizhu Tianma decoction combined with acupotomy.It is worthy of clinical promotion and application.
