中西医结合治疗卒中样神经白塞病回顾性分析

摘　　要：神经白塞病(Neuro-BD,NBD)是白塞病(Beh?et Disease,BD)罕见的最严重并发症之一,可致残或死亡,其中血管性NBD可表现为卒中样发作,不论何种类型NBD,其治疗主要分为急性发作期和预防复发期。目前在急性期西医多采取激素联合免疫抑制剂控制NBD病情,待病情稳定后长期予免疫抑制剂以降低复发可能,达到更有效的治疗效果。中医对于BD研究较多,但针对NBD研究较少。笔者从BD的病机出发,临证治疗1例卒中样发病的NBD患者,结合此病例的发病特点,在西医激素和免疫抑制剂治疗的基础上运用中医辨证论治,采取针灸疗法来治疗此病后遗症,重在调气、祛瘀、醒神。通过回顾性分析该患者的治疗过程,阐明治疗思路,抛砖引玉,为日后临床治疗此病拓展思路。Objective Neuro-Behcet’s disease(NBD)is one of the most serious complications of Behcet’s disease,which can cause disability or death.Of these,the vascular NBD may be manifested as stroke.Regardless of the type of NBD,the treatment is divided into the acute stage and the prevention of recurrence phase mainly.At present,in the acute stage,western medicine mostly adopts hormone combined with immunosuppressant to control the NBD condition,and immunosuppressant is given for a long time after the condition stabilizes to reduce the possibility of recurrence and achieve more effective treatment effect.However,there are many studies on Behcet’s disease in traditional Chinese medicine(TCM)but few on NBD.The authors have treated a NBD patient presenting as stroke with syndrome differentiation treatment of TCM on the basis of hormonal and infliximab treatment from the pathogenesis of Behcet’s disease,combined with the characteristics of this case.The emphasis is on regulating qi,removing stasis and awakening spirit.We illustrate the therapy idea of the case by the retrospective analysis to expand the clinical thinking of NBD in the future.
