帕博利珠单抗致降钙素原升高的用药分析

摘　　要：通过对帕博利珠单抗致降钙素原(PCT)升高的1例的用药分析,提示临床在使用帕博利珠单抗过程中重视其药物不良反应的密切监测。给予帕博利珠单抗后患者出现发热至38.9℃左右,给予复方对乙酰氨基酚片后患者体温恢复正常,无咳嗽咳痰、腹胀腹痛、尿频尿急等不适,血常规、C反应蛋白、尿常规、血培养等结果正常,仅PCT升高,考虑为输注帕博利珠单抗引起。临床医师解读PCT检测结果时应意识到其局限性,不能将单独的PCT升高作为使用抗菌药物的依据,考虑有非感染性因素引起PCT升高的可能,应结合临床情况综合评估PCT升高的原因。对于免疫功能异常的肿瘤患者,应加强药物不良反应的监测,保证患者用药安全。Through the analysis of a case report of elevation of procalcitonin(PCT) plasma levels caused by pembrolizumab, it is suggested that clinical medical personnel should pay attention to the monitoring of adverse reactions of pembrolizumab. After administration of pembrolizumab, the patient developed fever up to 38.9 °C and recovered after administration of compound paracetamol.There were no symptoms such as cough and sputum, abdominal distention and pain, frequent urination.The results of blood routine examination, C reaction protein, urine routine examination and blood culture were normal, only PCT was elevated, which was considered to be caused by infusion of pembrolizumab.The clinicians should be aware of the limitations of PCT test results and comprehensively evaluate the reasons for elevation of PCT plasma levels. For patients with abnormal immune function, the monitoring of adverse drug reactions should be strengthened to ensure the safety of drug use.
