摘 要:目的 探讨压力-应变环(PSL)评估非对称性肥厚型心肌病(HCM)局部心肌做功变化的应用价值。方法 分别选取25例非对称性HCM患者和健康体检者,比较两组左室整体有效功(GCW)、整体做功指数(GWI)、整体做功效率(GWE)、整体无效功(GWW)、纵向峰值应变离散度(PSD)的差异。根据室壁厚度将HCM患者所有心肌节段分为HCM1、HCM2、HCM3组,测量节段做功指数(SWI)、节段做功效率(SWE)。结果 HCM组较对照组GCW、GWI、GWE减低,GWW、PSD增加(P<0.05),HCM1、HCM2、HCM3组SWI、SWE较对照组降低(P<0.05),且随着室壁增厚,SWI、SWE降低更为明显,即HCM1>HCM2>HCM3。结论 PSL能定量分析HCM患者不同厚度室壁心肌做功变化,为无创评价左室收缩功能提供一种可靠的方法。Objective To explore the application value of pressure-strain loops(PSL)in assessment of the change of myocardial work in different hypertrophic sites in patients with asymmetric hypertrophic cardiomyopathy(HCM). Methods Twenty-five patients with asymmetric HCM and twenty-five healthy volunteers were consecutively included. Comparing the differences of the following parameters between the two groups: global constructive work(GCW), global work index(GWI), global work efficiency(GWE), global wasted work(GWW) and longitudinal peak strain dispersion(PSD). According to the wall thickness, all of myocardial segments in HCM patients were further divided into three subgroups: HCM1, HCM2, HCM3 groups and segmental work index(SWI), segmental work efficiency(SWE) were measured in each group. Results Compared with the control group, GCW, GWI, GWE were significantly reduced, while GWW, PSD were increased in the HCM group(P<0.05). With the wall thickening, SWI and SWE in HCM1, HCM2 and HCM3 groups decreased more significantly compared to the control group. The differences between each group were statistically significant(P<0.05). Conclusions PSL could quantitatively analyze the changes of myocardial work of different hypertrophic sites in patients with HCM, providing a valuable way to evaluate LV systolic function.
