超声引导下腱鞘囊肿介入治疗的临床价值探讨

摘　　要：目的 探讨超声引导下抽吸、粗针切割、挤压联合注射曲安奈德治疗腱鞘囊肿的临床应用价值。方法 选取经超声确诊的86例腱鞘囊肿按患者自我意愿随机分配入单纯抽液组26例及抽液、粗针切割、挤压联合曲安奈德组60例。依据不同方法进行治疗,治疗后12个月随访复查评价疗效。结果 26例单纯抽液组治疗有效12例(总有效率47.15%);60例抽液、粗针切割、挤压联合曲安奈德组治疗有效50例(总有效率83.3%),两组间疗效差异具有统计学差异(χ^(2)=15.68,P<0.05)。所有患者均未出现明显并发症。结论 超声引导下介入治疗腱鞘囊肿是一种安全、有效的方法,多点、多方向对囊壁及分隔处切割、挤压联合曲安奈德注射可提高腱鞘囊肿治疗效果。Objective To investigate the clinical value of ultrasound-guided suction, cutting, squeezing and injection of triamcinolone acetonide in the treatment of ganglion cysts. Methods Eighty-six cases of ganglion cysts diagnosed by high-frequency ultrasound were randomly assigned to the simple fluid aspiration group(n=26), fluid aspiration, thick needle cutting, squeezing, and combined triamcinolone acetonide group(n=60). Different methods were used for treatment, and 12 months after treatment, follow-up was conducted to evaluate the effectiveness of treatment. Results 12 cases(47.15%) were effective in the treatment of 26 cases in the simple fluid aspiration group, and 50 cases(83.3%) were effective in the treatment of 60 cases of fluid aspiration, thick needle cutting and squeezing combined with triamcinolone acetonide group. There was a statistically significant difference in efficacy between the two groups(χ^(2)=15.68, P<0.05). There were no obvious complications in all patients. Conclusions Ultrasound-guided interventional treatment of ganglion cysts is safe and reliable. Multi-point and multi-direction cutting of the cyst wall and separation with thick needle cutting, squeezing and triamcinolone acetonide injection can improve the therapeutic effect of ganglion cysts.
