锁骨钩钢板内固定加喙锁韧带修补术治疗肩锁关节脱位124例  

124 Cases of Clavicle Hook Plate Internal Fixation Combined with Repairof Coracoclavicular Ligament on the Treatment of Dislocation of Acromioclavicular Joint

摘　　要：目的:探讨锁骨钩钢板内固定加喙锁韧带修补术治疗肩锁关节脱位的临床疗效及对患者术后并发症的影响。方法:回顾性分析2018年1月至2021年8月采用锁骨钩钢板内固定加喙锁韧带修补术治疗的124例TossyⅢ型肩锁关节脱位患者的临床资料。统计患者的手术相关指标、疼痛视觉模拟(VAS)评分、肩锁关节功能恢复情况(Constant-Murley肩关节功能评分、肩外上举ROM、肩锁间隙、喙锁间隙)、临床疗效及术后并发症发生情况。结果:所有患者均顺利完成手术,术中无血管、臂丛神经损伤等严重并发症。患者手术时间为(60.06±6.01)min,术中出血量为(99.23±9.92)mL,术后住院时间为(11.18±1.12)d。124例患者均获得随访,随访时间6个月。末次随访时,患者VAS评分、肩锁间隙和喙锁间隙均较术前减小,Constant-Murley肩关节功能评分量表中各项目评分、肩外上举ROM均较术前升高,差异有统计学意义(P<0.05)。根据Karlsson标准,优良率为92.74%(115/124)。术后并发症发生率为6.45%(8/124)。结论:TossyⅢ型肩锁关节脱位使用锁骨钩钢板内固定加喙锁韧带修补术治疗效果较好,能恢复患者肩关节功能,且并发症发生率较低。Objective:To investigate the clinical efficacy of internal fixation of clavicle hook plate combined with coracoclavicular ligament repair on the treatment of acromioclavicular joint dislocation and the effect on postoperative complications of patients.Methods:The clinical data of 124patients with Tossy typeⅢacromioclavicular dislocation treated by internal fixation of clavicular hook plate combined with rostral ligament repair from January 2018to August 2021were retrospectively analyzed.The surgery-related indexes,pain visual analogue scale(VAS)scores,functional recovery of the acromioclavicular joint(Constant-Murley shoulder function scores,external shoulder supination ROM,acromioclavicular gap,coracoclavicular gap),clinical efficacy and postoperative complications were counted.Results:All patients completed the surgery successfully without serious intraoperative complications such as vascular and brachial plexus nerve injuries.The operative time was(60.06±6.01)min;intraoperative bleeding was(99.23±9.92)mL,and postoperative hospital stay was(11.18±1.12)d.All 124patients were followed up for 6months.At the final follow-up,the VAS scores,acromioclavicular gap and coracoclavicular gap were reduced compared with those before surgery,and the scores of all patients in the Constant-Murley shoulder function rating scale and ROM of shoulder abduction were higher than those before surgery(P<0.05).The excellent rate was 92.74%(115/124)according to Karlsson's criteria.The postoperative complication rate was 6.45%(8/124).Conclusion:Tossy typeⅢacromioclavicular dislocation treated by internal fixation of the clavicle hook plate combine with coracoclavicular ligament repair has good efficacy and effectively restore the function of the patient's shoulder joint with a low complication rate.
