养血饮与环孢素A治疗再生障碍性贫血的临床效果

摘　　要：目的 探讨养血饮与环孢素A治疗再生障碍性贫血患者的临床效果。方法 选取2017年5月—2020年5月本院再生障碍性贫血患者50例,按随机数字表法分为联合用药组25例和单一用药组25例,单一用药组采用环孢素A治疗,联合用药组在单一用药组基础上采用养血饮治疗。对比两组疗效、骨髓增生度、治疗前后血红蛋白和血小板水平及血清γ干扰素(IFN-γ)、白细胞介素6(IL-6)水平的变化。结果 联合用药组总有效率为92.00%(23/25),与单一用药组的76.00%(19/25)相比,差异无统计学意义(χ^(2)=1.339,P=0.2056);联合用药组骨髓增生度与单一用药组差异无统计学意义(Z=1.611,P=0.107);治疗后,联合用药组患者血红蛋白(90.46±15.61)和血小板(80.63±14.48)水平均较单一用药组(77.45±11.32、60.79±12.68)高,差异有统计学意义(t=3.374、5.154,P<0.001);治疗后,联合用药组血清IFN-γ(142.28±28.66)、IL-6(31.63±3.42)水平较单一用药组(175.66±29.14、37.43±4.12)低,差异有统计学意义(t=4.090、5.416,P<0.001)。结论 采用养血饮与环孢素A对治疗再生障碍性贫血患者的临床效果确切,可调节免疫,减轻炎性反应,改善骨髓增生度和贫血状况,值得临床推广应用。Objective To explore the clinical effect of Yangxue Decoction and cyclosporine A in the treatment of patients with aplastic anemia.Methods A total of 50 patients with aplastic anemia in our hospital from May 2017 to May2020 were randomly divided into a combined group and a monotherapy group, with 25 cases in each group. The monotherapy group was treated with cyclosporine A, and the combined group was treated with Yangxue Decoction on the basis of the monotherapy group. The curative effect, bone marrow hyperplasia, levels of hemoglobin and platelet, and serum levels of γ interferon(IFN-γ) and interleukin-6(IL-6) were compared between the two groups before and after the treatment.Results The total effective rate of the combination group was 92.00%(23/25), and the effective rate of the monotherapy group was 76.00%(19/25), and there was no significant difference between the two groups(χ^(2)= 1.339, P =0.2056). The bone marrow hyperplasia degrees was no significant difference between the two group(Z = 1.611, P = 0.107). After treatment, the levels of hemoglobin(90.46 ± 15.61) and platelet(80.63 ± 14.48) in the combination group were higher than those in the monotherapy group:(77.45 ± 11.32) and(60.79 ± 12.68)(t = 3.374, 5.154, P < 0.05). After treatment, the serum levels of IFN-γ and IL-6 in the combination group were(142.28 ± 28.66) and(31.63 ± 3.42) respectively, which were significantly lower than those in the monotherapy group:(175.66 ± 29.14) and(37.43 ± 4.12)(t = 4.090, 5.416, P < 0.05).Conclusion The clinical effect of Yangxue Decoction and cyclosporine A in the treatment of patients with aplastic anemia is definite. It can regulate immunity,reduce inflammatory reactions, and improve bone marrow hyperplasia and anemia. It is worthy of clinical application.
