麻黄附子细辛汤治疗偏头痛的临床研究

摘　　要：目的:观察麻黄附子细辛汤治疗偏头痛的临床疗效。方法:将80例偏头痛患者按照随机数字表法分为对照组和观察组,每组各40例。对照组给予头痛宁胶囊治疗,观察组在对照组治疗的基础上加用麻黄附子细辛汤。比较两组患者的临床疗效及治疗前后头痛评分、头痛发作频率、每次发作持续时间、中医证候积分、匹兹堡睡眠质量指数量表(pitts-burgh sleep quality index,PSQI)评分以及血清生化指标[5-羟色胺(5-hydroxy tryptamin,5-HT)、内皮素-1(Endothelin-1,ET-1)]变化情况。结果:观察组有效率为92.50%,对照组有效率为75.00%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后头痛评分、头痛发作频率、每次发作持续时间、中医证候积分及PSQI评分均低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后血清ET-1水平低于本组治疗前,5-HT高于本组治疗前,治疗后观察组血清ET-1水平低于对照组,5-HT高于对照组,差异具有统计学意义(P<0.05)。结论:麻黄附子细辛汤治疗偏头痛,能有效缓解头痛症状,减少发作频率及发作持续时间,从而改善睡眠质量。Objective:To observe the clinical efficacy of Ephedra and Aconite and Asarum Decoction on migraine.Methods:Accord-ing to the random number table method,80 migraine patients were divided into the observation group and the control group,with 40 cases in each group.The control group was treated with Headache-Removing Capsules,while the observation group was treated with Ephedra and Aconite and Asarum Decoction on the basis of the treatment of the control group.The clinical efficacy and changes of headache score,headache attack frequency,duration of each attack,TCM syndrome scores,Pittsburgh sleep quality index(PSQI)score and serum biochemical indexes[including 5-hydroxytryptamine(5-HT),endothelin-1(ET-1)]were compared before and after treatment between the two groups.Results:The effective rate was 92.50%in the observation group and 75.00%in the con-trol group.The difference between the two groups was statistically significant(P<0.05).After treatment,the headache score,headache attack frequency,duration of each attack,TCM syndrome scores and PSQI score of the two groups were lower than those of the same group before treatment,and the above indexes of the observation group were lower than those of the control group after treatment,and all the differences were statistically significant(P<0.05).After treatment,the level of serum ET-1 in the two groups was lower than that before treatment,while 5-HT was higher than that before treatment.The level of serum ET-1 in the ob servation group was lower than that in the control group,while 5-HT was higher than that in the control group,and all the differ-ences were statistically significant(P<0.05).Conclusion:Ephedra and Aconite and Asarum Decoction can effectively alleviate the symptoms of migraine,reduce the frequency and duration of attacks,and improve the quality of sleep.
