消溃方灌肠治疗溃疡性结肠炎湿热内蕴证临床研究

摘　　要：目的:观察消溃方灌肠治疗溃疡性结肠炎湿热内蕴证的临床疗效。方法:选择2019年6月至2021年6月郑州人民医院肛肠科治疗的溃疡性结肠炎湿热内蕴证患者48例,按照随机数字表法分为治疗组和对照组,每组24例。治疗组给予消溃方水煎液灌肠治疗,对照组给予康复新液灌肠治疗。比较两组患者治疗前后腹泻、腹痛、黏液便、脓血便、里急后重等临床症状积分改善情况、血清白细胞介素(interleukin,IL)-6、IL-10及肿瘤坏死因子(tumor necrosis factor,TNF)-α,DAI评分/Baron评分的变化情况,并对比两组患者临床疗效。结果:治疗组治疗后临床症状积分低于对照组,差异有统计学意义(P<0.05);治疗组治疗后IL-6、IL-10及TNF-α水平低于对照组,差异有统计学意义(P<0.05);治疗组治疗后DAI评分、Baron肠镜黏膜组织学评分低于对照组,差异有统计学意义(P<0.05);治疗组有效率为83.3%,对照组有效率为62.5%,治疗组有效率高于对照组,差异有统计学意义(P<0.05)。结论:消溃方可有效改善溃疡性结肠炎湿热内蕴证患者腹泻、腹痛、黏液便、脓血便、里急后重等症状,并可调节炎症因子IL-6、IL-10及TNF-α的水平,缓解肠道炎症反应。Objective:To observe the clinical efficacy of enema with Decoction for Removing Ulcerative Colitis on ulcerative colitis with syndrome of internal retention of dampness-heat.Methods:A total of 48 patients with ulcerative colitis with syndrome of inter-nal retention of dampness-heat treated in The Anorectal Department of People's Hospital of Zhengzhou from June 2019 to June 2021 were randomly divided into the treatment group and the control group,with 24 cases in each group.The treatment group was given enema with Decoction for Removing Ulcerative Colitis,while the control group was given enema with New Recovery Liquid.The improvement of clinical symptom scores,such as diarrhea,abdominal pain,mucus stool,purulent blood stool,tenesmus and etc.,and the changes of serum interleukin(IL)-6,IL-10 and tumor necrosis factor(TNF)-αbefore and after treatment,and the clinical efficacy of the two groups were compared between the two groups.Results:After treatment,the clinical symptom scores of the treatment group were lower than those of the control group,and all the differences were statistically significant(P<0.05).The levels of IL-6,IL-10 and TNF-αin the treatment group were lower than those of the control group,and all the differences were sta-tistically significant(P<0.05).The DAI score and Baron enteroscopy mucosal histological score of the treatment group were low-er than those of the control group,and all the differences were statistically significant(P<0.05).The effective rate of the treat-ment group was 83.3%,which was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Decoction for Removing Ulcerative Colitis can effectively improve the symptoms of diarrhea,abdominal pain,mu-cinous stool,purulent blood stool,tenesmus,severe diarrhea and other symptoms of patients with ulcerative colitis with syndrome of internal retention of dampness-heat,and regulate inflammatory factors IL-6,IL-10 and TNF-α,and alleviate intestinal inflamma-tion.
