实脾饮合真武汤加减治疗慢性肾炎水肿脾肾阳虚证临床研究

摘　　要：目的:观察实脾饮合真武汤加减治疗慢性肾炎水肿脾肾阳虚证的临床疗效。方法:选取2018年7月至2019年11月广州市黄埔区中医院收治的慢性肾炎患者74例,按照随机数字表法分为对照组和观察组,每组37例。对照组给予常规西医治疗,观察组在对照组治疗的基础上给予实脾饮合真武汤加减治疗。观察两组患者临床疗效、不良反应发生率,比较两组患者治疗前后中医证候积分、肾功能相关指标[24 h尿蛋白定量、血肌酐(serum creatinine,Scr)、血浆白蛋白、血尿素氮(blood urea nitrogen,BUN)]、免疫球蛋白相关指标[血清免疫球蛋白(Immunoglobulin,Ig)A、IgG、IgM及补体3(complement 3,C3)]。结果:观察组有效率为97.30%,对照组有效率为81.08%,观察组有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后中医证候积分、24 h尿蛋白定量、Scr、BUN水平低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗后IgA、IgG、IgM及补体C3水平高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为10.82%,对照组不良反应发生率为13.52%,观察组低于对照组,但差异无统计学意义(P>0.05)。结论:实脾饮合真武汤加减治疗慢性肾炎水肿脾肾阳虚证,能减轻患者临床症状,改善肾功能。Objective:To observe the clinical efficacy of Decoction for Reinforcing the Spleen combined with Modified True Warrior Decoction on chronic nephritis edema with spleen-kidney yang deficiency syndrome.Methods:A total of 74 patients with chronic nephritis treated in Huangpu District Hospital of Traditional Chinese Medicine of Guangzhou from July 2018 to November 2019 were randomly divided into the control group and the observation group,with 37 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Decoction for Reinforcing the Spleen combined with Modified True Warrior Decoction on the basis of the treatment of the control group.The clinical efficacy and the incidence of adverse reactions of the two groups of patients were observed.The TCM syndrome scores,renal function related indexes[24h uri-nary protein quantification,serum creatinine(Scr),plasma albumin,blood urea nitrogen(BUN)],immunoglobulin related inde-xes[serum immunoglobulin(Ig)A,IgG,IgM and complement 3(C3)]were compared before and after treatment.Results:The effective rate of the observation group was 97.30%,which was higher than that of the control group:81.08%,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome scores,24-h urinary protein quantity,Scr and BUN levels of the two groups were lower than those before treatment,and the above indexes of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05).The levels of IgA,IgG,IgM and C3 in the observation group were significantly higher than those in the control group,and all the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 10.82%,which was lower than that in the control group:13.52%,but the difference was not statistically significant(P>0.05).Conclusion:Decoction for Reinforcing the Spleen combined with Modified True Warrior Decoction can alleviate the
