摘　　要：目的比较经口留取诱导痰液和肺泡灌洗液在病毒混合感染儿童重症肺炎支原体肺炎(MPP)中的诊断价值。方法将2019年1月—2020年12月期间诊断为重症MPP且进行肺泡灌洗治疗的患儿纳入研究,所有患儿均在行纤维/电子支气管镜检查当日进行经口留取诱导痰液(痰液),所得痰液及肺泡灌洗液同时送检病毒多重检测,对比2种标本病毒检出情况。结果共纳入198例MPP患儿,痰液标本检出病毒混合感染阳性52例(26.3%),主要为鼻病毒(30例,57.7%),腺病毒、副流感病毒(各5例,9.6%)。肺泡灌洗液标本检出病毒混合感染阳性96例(48.5%),主要为腺病毒(30例,31.3%)、副流感病毒(19例,19.8%)、呼吸道合胞病毒(15例,15.3%)。肺泡灌洗液标本的腺病毒检出率高于痰液(P<0.05)。结论肺泡灌洗液病毒多重检测腺病毒、副流感病毒等阳性率明显高于经口留取诱导痰液。Objective To compare the value of oral induction of sputum and bronchoalveolar lavage fluid(BALF)in detection of severe mycoplasma pneumoniae pneumonia(MPP)with virus mixed infection in children.Methods The children diagnosed as severe MPP and treated with alveolar lavage from January 2019 to December 2020 were included in the study.All the children underwent oral induction of sputum on the day of fiberoptic/electronic bronchoscopy,and the sputum and BALF were used for viruses multiple detection.The detection rate of the two specimens was compared.Results A total of 198 children with MPP were enrolled.Mixed infection was detected in 52 cases(26.3%).The top three pathogens were rhinovirus(30 cases),adenovirus(5 cases),parainfluenza virus(5 cases)and respiratory syncytial virus(3 cases),respectively(57.7%).Multiple detection results of BALF virus showed that 96 cases(48.5%)had mixed infection,and the top three pathogens were adenovirus(30 cases,31.3%),parainfluenza virus(19 cases,19.8%)and respiratory syncytial virus(15 cases,15.3%).The detection rate of adenovirus and rhinovirus in BALF was higher than that in oral induced sputum(P<0.05).Conclusion The mixed infection virus detection rate of BALF is more higher and clearer.
