摘 要:目的:分析可能肌肉衰减症和肌肉衰减症前期老年人功能性体适能的差异,探寻可能肌肉衰减症是否更有利于早期发现和预防老年肌肉衰减症及其危害,为防治肌肉衰减症提供实验依据。方法:以天津市社区老年人为研究对象,共募集和检测830例受试者,采用老年功能性体适能测试方案(SFT)进行各项指标测试。按照欧洲肌肉衰减症工作组(EWGSOP)的概念和亚洲肌肉衰减症工作组2019(AWGS2019)的诊断标准将研究对象分为非肌肉衰减症组(n=526,M/F:180/346)、肌肉衰减症前期组(n=46,M/F:17/29)和可能肌肉衰减症组(n=81,M/F:20/61),比较各组间功能性体适能的差异。结果:可能肌肉衰减症组的30 s屈臂测试(P<0.01)、30 s座椅起坐测试(P<0.01)、计时起立走(TUG)测试(P<0.05)和6 min步行测试(P<0.01)结果显著差于肌肉衰减症前期组。男性可能肌肉衰减症组6 min步行距离明显低于肌肉衰减症前期组,差异具有统计学意义(P<0.05);女性可能肌肉衰减症组30 s屈臂测试(P<0.01)、30 s座椅起坐测试(P<0.01)、6 min步行测试(P<0.01)、TUG测试(P<0.05)和反应时测试(P<0.05)结果均显著差于肌肉衰减症前期组。结论:可能肌肉衰减症老年人四肢肌力、动态平衡及敏捷性和有氧耐力显著低于肌肉衰减症前期老年人,从功能性体适能视角表明低握力作为可能肌肉衰减症定义更具健康风险的提示意义;可能肌肉衰减症新定义对进一步探讨肌肉衰减症发生发展机制和指导肌肉衰减症防治、预防其危害有重要价值。可能肌肉衰减症老年人性别不同,各项功能性体适能下降也有所不同,提示针对不同性别老年人的肌肉衰减症防治方法也应有所不同。Objective To analyze the differences in functional fitness between the elderly with"possible sarcopenia"and"pre-sarcopenia",and explore whether"possible sarcopenia"is more conducive to early prevention and detection of sarcopenia and its harm,so as to provide experimental evidences for the prevention and treatment of sarcopenia.Methods A total of 830 community-dwelling old adults in Tianjin were recruited and tested using the Senior Fitness Test(SFT).According to the concept proposed by European Working Group on Sarcopenia in Older People(EWGSOP)and the diagnostic criteria put forward by Asian Working Group for Sarcopenia 2019(AWGS 2019),they were divided into a non-sarcopenia group(n=526,M/F:180/346),a pre-sarcopenia group(n=46,M/F:17/29)and a possible sarcopenia group(n=81,M/F:20/61).The differences in functional physical fitness were compared among the 3 groups.Results The results of the 30-second arm curl test(P<0.01),30-second chair stand test(P<0.01),timed-up-and-go(TUG)test(P<0.05),and 6-minute walk test(P<0.01)of the possible sarcopenia group were significantly worse than the pre-sarcopenia group for both sexes.The average 6-minute walk distance among the male of the possible sarcopenia group were significantly shorter than per-sarcopenia group(P<0.05).The results of the 30-second arm curl test(P<0.01),30-second chair stand test(P<0.01),6-minute walk test(P<0.01),TUG test(P<0.05)and reaction time test(P<0.05)for the females in the possible sarcopenia group were significantly worse than the pre-sarcopenia group.Conclusion The muscles strength,dynamic balance,agility and aerobic endurance of the elderly adults with possible sarcopenia were significantly lower than those with pre-sarcopenia.From the perspective of functional physical fitness,it was confirmed that low grip strength should be a included in the definition of possible sarcopenia.Such new definition of possible sarcopenia is of great value to further explore the its mechanism of occurrence and development,as well as its prevention and treatment.T
