摘 要:目的 探究糖尿病足感染(DFI)患者病原菌分布特征及创面组织中细胞自噬相关蛋白表达。方法 选取2015年11月-2019年11月医院收治的196例DFI患者(感染期126例,感染控制期70例),分析患者病原菌分布,检测微管结合轻链蛋白-3(LC3)、自噬特异性基因(Beclin-1)和泛素结合蛋白(P62)的表达量和白细胞计数(WBC)、C-反应蛋白(CRP)、降钙素原(PCT)水平,分析检测结果。结果 126例感染期DFI患者共培养分离病原菌147株,包括革兰阴性菌84株、革兰阳性菌52株、真菌11株,主要病原菌为铜绿假单胞菌、奇异变形菌、金黄色葡萄球菌;感染期患者LC3、Beclin-1表达水平均低于感染控制期患者,P62表达水平和WBC、CRP、PCT水平均高于感染控制期患者,差异有统计学意义(P<0.05);重度感染患者LC3、Beclin-1表达低于中度、轻度感染患者,P62表达高于中度、轻度感染患者,中度感染患者LC3、Beclin-1表达低于轻度感染患者,P62表达高于轻度感染患者,差异均有统计学意义(P<0.05)。结论 DFI患者感染病原菌主要为革兰阴性菌,常见菌为铜绿假单胞菌、奇异变形菌、金黄色葡萄球菌,感染后DFI患者LC3、Beclin-1表达降低而感染指标水平和P62表达提高,且三者表达水平与感染程度有关。OBJECTIVE To investigate the distribution characteristics of pathogenic bacteria in patients with diabetic foot infection(DFI) and the expression of autophagy-related proteins in wound surface tissues. METHODS Total of 196 patients with DFI(126 cases in the infection period, 70 cases in the infection control period) who were treated in hospital between Nov 2015 and Nov 2019 were recruited. The distribution of pathogenic bacteria was analyzed, and the expression levels of microtubule-associated protein1 light chain 3(LC3), autophagy-specific gene(Beclin-1) and ubiquitin-binding protein(P62) and levels of white blood cell count(WBC), C-reactive protein(CRP) and procalcin(PCT) were detected and the results were analyzed. RESULTS Among 126 patients with DFI during infection period, 147 strains of pathogenic bacteria were co-cultured and isolated, including 84 strains of Gram-negative bacteria, 52 strains of Gram-positive bacteria and 11 strains of fungi. The main pathogenic bacteria were Pseudomonas aeruginosa, Proteus mirabilis and Staphylococcus aureus. The expression levels of LC3 and Beclin-1 of patients in the infection period were significantly lower than those in patients of the infection control group, while the levels of P62 expression, WBC, CRP and PCT were significantly higher than those in patients during infection control period(P<0.05). In the infection group, the expression of LC3 and Beclin-1 in severe patients were significantly lower than those in moderate and mild patients, while the expression of P62 was higher. And the expressions of LC3 and Beclin-1 in moderate patients were significantly lower than those in mild patients, while the expression of P62 was significantly higher than that in mild patients(P<0.05). CONCLUSION The pathogenic bacteria in patients with DFI are mainly Gram-positive bacteria, and the common bacteria are P. aeruginosa, P. mirabilis and S. aureus.
