摘 要:目的 分析武汉地区宫颈病变妇女人乳头瘤病毒(HPV)感染及人类白细胞抗原(HLA)-DRB1、DQA1基因多态性的关联性。方法 选择2019年1月-2019年12月武汉市第三医院收治的200例宫颈病变患者为研究对象,统计武汉地区宫颈病变妇女HPV感染情况,检测HLA-DRB1、HLA-DQA1基因多态性,分析HLA-DRB1、HLA-DQA1基因多态性与武汉地区宫颈病变妇女HPV感染的关联性。结果 200例宫颈病变患者中,感染HPV的宫颈病变患者173例,感染率为86.50%;宫颈上皮细胞瘤变患者HPV感染率低于宫颈癌患者(P<0.05);173例宫颈病变HPV感染患者中,包括高危型HPV感染患者104例和低危型HPV感染患者69例,高危型HPV感染患者以HPV-52、HPV-16和HPV-58感染为主,低危型HPV感染患者以HPV-6感染为主;HPV感染组HLA-DRB1*07等位基因频率高于对照组(P<0.05),HLA-DRB1*13等位基因频率低于对照组(P<0.05);HPV感染组HLA-DQA1*0102等位基因频率高于对照组(P<0.05),HLA-DQA1*0501等位基因频率低于对照组(P<0.05)。结论 武汉地区宫颈病变妇女HPV感染以HPV-52、HPV-16、HPV-58和HPV-6、HPV-11感染为主,HLA-DRB1*07、HLA-DQA1*0102等位基因可能是宫颈病变患者HPV感染的易感基因,HLA-DRB1*13、HLA-DQA1*0501等位基因可能是宫颈病变患者HPV感染的保护基因。OBJECTIVE To analyze the association between human papillomavirus(HPV) infection and human leukocyte antigen(HLA)-DRB1 and DQA1 gene polymorphisms in women with cervical lesions in Wuhan area. METHODS A total of 200 patients with cervical lesions admitted to Wuhan Third Hospital between Jan 2019 and Dec 2019 were enrolled. The HPV infection status of women with cervical lesions in Wuhan area were statistically analyzed and HLA-DRB1 and HLA-DQA1 gene polymorphisms were detected, and the association between gene polymorphisms of HLA-DRB1 and HLA-DQA1 and HPV infection was analyzed among women with cervical lesions in Wuhan area. RESULTS Among the 200 patients with cervical lesions, 173 cases were infected with HPV, with the infection rate of 86.50%. The HPV infection rate in patients with cervical epithelial neoplasia was significantly lower than that in patients with cervical cancer(P<0.05). Among 173 patients with cervical lesions and HPV infection, 104 cases were infected with high-risk HPV types and 69 cases with low-risk HPV types. The patients with high-risk HPV infection were mainly HPV-52, HPV-16 and HPV-58 serotypes, and the low-risk HPV infections in patients were mainly HPV-6 serotype. The frequency of HLA-DRB1*07 allele in the HPV infection group was significantly higher than that in the control group(P<0.05), while the frequency of HLA-DRB1*13 allele was significantly lower than that in the control group(P<0.05). The frequency of HLA-DQA1*0102 allele in the HPV infection group was significantly higher than that in the control group(P<0.05) while the frequency of HLA-DQA1*0501 allele was significantly lower than that in the control group(P<0.05). CONCLUSION The types of HPV infecting cervix in women with cervical lesions in Wuhan area were mainly HPV-52, HPV-16, HPV-58, HPV-6 and HPV-11. HLA-DRB1*07 and HLA-DQA*10102 alleles may be susceptibility genes for HPV infection in patients with cervical lesions, and HLA-DRB1*13 and HLA-DQA1*0501 alleles may be protective genes for HPV infection in patients with
