摘 要:目的 探讨白细胞介素-1A(IL-1A)和白细胞介素-1B(IL-1B)和白细胞介素-6(IL-6)基因多态性与妊娠期巨细胞病毒(CMV)感染相关性。方法 选择海南医学院第一附属医院2019年1月-2020年8月收治的CMV感染自然流产孕妇80例为研究对象,根据CMV-DNA检测结果将其分为CMV-DNA阳性组(n=26)和CMV-DNA阴性组(n=54)。入院时抽取其外周血采用聚合酶链式扩增反应检测CMV-DNA水平;采用限制性片段长度多态性聚合酶链反应检测IL-1A基因899启动子、IL-1B基因511启动子和IL-6基因572启动子多态性;采用酶联免疫吸附法检测血清中IL-1A、IL-1B和IL-6水平。结果 两组孕妇IL-1A-899、IL-1B-511和IL-6-572基因型分布在理论值与实际值之间比较差异无统计学意义,具有群体代表性;CMV-DNA阳性组与CMV-DNA阴性组IL-1A-899、IL-1B-511位点多态性基因型分布比较差异无统计学意义,CMV-DNA阳性组IL-6-572位点GG基因型、G等位基因频率高于CMV-DNA阴性组(P<0.05);CMV-DNA阳性组患者血清中IL-1A、IL-1B和IL-6水平高于CMV-DNA阴性组(P<0.05);GG型IL-6水平高于CG型和CC型(P<0.05)。结论 IL-6-572位点GG型增加本地区女性CMV活动性感染易感性,可能与自然流产发生率增加有关。OBJECTIVE To explore the relationship between interleukin-1 A(IL-1 A), interleukin-1 B(IL-1 B) and interleukin-6(IL-6) gene polymorphisms and human cytomegalovirus(CMV) infection during pregnancy. METHODS Total of 80 pregnant women with CMV infection and spontaneous abortion admitted to the First Affiliated Hospital of Hainan Medical University from Jan 2019 to Aug 2020 were recruited and divided into the CMV-DNA positive group(26 cases) and CMV-DNA negative group(54 cases) according to the CMV-DNA test results. Group peripheral blood collected on admission was used to detect CMV-DNA level by polymerase chain amplification reaction. Restriction fragment length polymorphism polymerase chain reaction was used to detect polymorphisms of IL-1 A gene 899 promoter, IL-1 B gene 511 promoter and IL-6 gene 572 promoter. Enzyme-linked immunosorbent assay was used to detect the levels of IL-1 A, IL-1 B and IL-6 in serum. RESULTS There was no significant difference between the theoretical value and the actual value of the distribution of IL-1 A-899, IL-1 B-511, and IL-6-572 genotypes in the two groups of pregnant women, indicating population representation. There was no significant difference in the genotype distribution of IL-1 A-899 and IL-1 B-511 polymorphisms between the CMV-DNA positive group and the CMV-DNA-negative group. The GG genotype and G allele at IL-6-572 in the CMV-DNA positive group were significantly higher than those in the DNA-negative group(P<0.05). The serum levels of IL-1 A, IL-1 B and IL-6 in the CMV-DNA positive group were significantly higher than those in the CMV-DNA negative group(P<0.05);the level of IL-6 in GG type was significantly higher than that in type CG and CC(P<0.05). CONCLUSION IL-6-572 locus GG type significantly increases the susceptibility of CMV infection in women in this area, which may be related to the increased incidence of spontaneous abortion.
