摘 要:目的 探讨小儿肺炎支原体肺炎(MPP)血清白细胞介素(IL)-25、IL-33及嗜酸细胞趋化因子(Eotaxin)与肺功能的相关性。方法 选取2020年1月-2021年1月郑州大学第三附属医院收治的120例MPP患儿为MPP组,另选取同期医院体检的年龄相仿的健康儿童120名为对照组。收集两组一般资料,检测两组的肺功能指标[潮气量(VT)、达峰时间比(tPTEF/TE)及呼出75%潮气量时的瞬间流速与潮气呼气峰流速比(TEF25/PTEF%)]及血清炎症因子[肿瘤坏死因子-α(TNF-α)、IL-4、IL-25、IL-33]、嗜酸性粒细胞(EOS)相关因子水平[嗜酸性粒细胞阳离子蛋白(ECP)、半胱氨酰白三烯(CysLTs)、Eotaxin]。采用Pearson相关性分析MPP患儿血清IL-25、IL-33及Eotaxin与肺功能指标及三者间的相关性。结果 MPP组患儿TNF-α、IL-4、IL-25及IL-33各炎症因子指标水平均高于对照组(P<0.05)。MPP组患儿ECP、CysLTs及Eotaxin各EOS相关因子指标水平均高于对照组(P<0.05)。MPP组患儿VT、tPTEF/TE及TEF25/PTEF%各肺功能指标水平均低于对照组(P<0.05)。Pearson相关性分析结果显示,IL-25、IL-33及Eotaxin均与VT、tPTEF/TE及TEF25/PTEF%各肺功能指标呈负相关关系(P<0.05);且IL-25、IL-33均与Eotaxin呈正相关关系(P<0.05)。结论 MPP患儿血清IL-25、IL-33及Eotaxin水平存在异常升高,且与患儿肺功能情况关系密切,三者在MPP的发生发展中扮演了重要角色。OBJECTIVE To explore the association between serum interleukin(IL)-25, IL-33, eotaxin(Eotaxin) and lung function of the children with Mycoplasma pneumoniae pneumonia(MPP). METHODS A total of 120 children with MPP who were treated in the Third Affiliated Hospital of Zhengzhou University from Jan 2020 to Jan 2021 were assigned as the MPP group, meanwhile, 120 healthy children of the same age who received physical examination were chosen as the control group. The baseline data were collected, the lung function indexes [tidal volume(VT), ratio of time to peak(tPTEF/TE), and the ratio of instantaneous flow rate to tidal peak expiratory flow rate at 75% tidal volume exhalation(TEF25/PTEF%)], serum inflammatory factors [tumor necrosis factor-α(TNF-α), IL-4, IL-25, IL-33], eosinophils(EOS)-related factors [eosinophil cationic protein(ECP), cysteinyl leukotrienes(CysLTs), Eotaxin] were detected. The association between the serum IL-25, IL-33, Eotaxin and the lung function indexes as well as the relationship among the three indexes was observed by Pearson correlation analysis. RESULTS The levels of serum inflammatory factors TNF-α, IL-4, IL-25 and IL-33 of the MPP group were significantly higher than those of the control group(P<0.05). The levels of EOS-related factors ECP, CysLTs and Eotaxin of the MPP group were significantly higher than those f the control group(P<0.05).The levels of lung function indexes VT, tPTEF/TE and TEF25/PTEF% of the MPP group were significantly lower than those of the control group(P<0.05). Pearson correlation analysis showed that the IL-25, IL-33 and Eotaxin were negatively correlated with the lung function indexes VT, tPTEF/TE and TEF25/PTEF%(P<0.05);the IL-25 and IL-33 were positively correlated with the Eotaxin(P<0.05). CONCLUSION The MPP children show abnormal rise of serum IL-25, IL-33 and Eotaxin, which is closely associated with the lung function, and the three indexes play an important role in genesis and development of MPP.
