摘  要:目的 探讨牙种植修复术后种植体周围感染对核苷酸结合寡聚化结构域样受体3(NLRP3)信号通路及炎症细胞因子的影响。方法 从2017年1月-2020年1月海口市第三人民医院531例行牙种植体修复术患者中,选择术后并发种植体周围感染的48例患者为感染组,按1∶1病例对照分析比例选取未并发种植体周围感染的48例患者为非感染组;采集两组患者的空腹静脉血,分离外周血单核细胞并分别采用实时荧光定量聚合酶链反应和蛋白质印迹法检测NLRP3、半胱氨酸天冬氨酸蛋白水解酶1(Caspase-1)、白细胞介素1β(IL-1β)的mRNA和蛋白表达;采集两组患者龈沟液标本,采用酶联免疫吸附法检测IL-1β、白细胞介素-6(IL-6)水平。结果 48例口腔种植体周围细菌感染患者共培养分离病原菌64株,其中厌氧菌51株(79.69%),需氧菌13株(20.31%);感染组患者PMBCs中NLRP3、Caspase-1、IL-1βmRNA和蛋白水平均高于非感染组(P<0.05);种植体周围炎和种植体周围黏膜炎患者龈沟液IL-1β、IL-6水平均高于非感染组患者(P<0.05),种植体周围炎患者龈沟液IL-1β、IL-6水平高于种植体周围黏膜炎患者(P<0.05)。结论 NLRP3信号通路参与了牙种植体感染后种植体周围病的发生发展过程,有助于协助早期诊断种植体周围病。OBJECTIVE To explore effect of peri-implant infection after dental implant repair on nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3) signaling pathway and inflammatory cytokines levels. METHODS A total of 531 patients undergoing dental implant restoration in the Haikou Third People′s Hospital from Jan 2017 to Jan 2020 were recruited in this study. 48 patients with postoperative peri-implant infection were divided into the infection group, and 48 patients without peri-implant infection were in the non-infection group. The fasting venous blood was collected to isolate peripheral blood mononuclear cells(PBMCs). The mRNA and proteins of NLRP3, cysteinyl aspartate specific proteinase 1(Caspase-1) and interleukin 1β(IL-1β) were detected by real-time quantitative polymerase chain reaction and Western blot assays. The samples of gingival crevicular fluid(GCF) from both groups were collected. The levels of IL-1β and interleukin 6(IL-6) were detected by enzyme-linked immunosorbent assay. RESULTS 64 strains of pathogens were isolated from 48 patients with peri-implant infection, including 51 strains(79.69%) of anaerobes and 13 strains(20.31%) of aerobes. The mRNA and protein levels of NLRP3, Caspase-1 and IL-1β in PBMCs in the infection group were significantly higher than those in the non-infection group(P<0.05). The levels of IL-1β and IL-6 in GCF of patients with peri-implantitis and peri-implant mucositis were significantly higher than those in the non-infection group(P<0.05). The levels of IL-1β and IL-6 in GCF of patients with peri-implantitis were significantly higher than those with peri-implant mucositis(P<0.05). CONCLUSION NLRP3 signaling pathway is involved in the occurrence and development of peri-implant diseases after dental implant infection, which is conducive to early diagnosis of peri-implant diseases.
