摘　　要：目的探究尖锐湿疣(CA)初发、复发患者外周血人乳头状瘤病毒(HPV)16抗原特异性CD_(8)^(+)细胞毒性T细胞(CTL)表型特点及其与HPV DNA的关系。方法选择海口市人民医院2017年10月-2020年10月收治的90例CA患者为研究对象,依据发病情况分为初发组43例和复发组47例,选择近期于医院接受包皮或子宫肌瘤手术患者30例为对照组,测定并比较外周血HPV16抗原特异性CD_(8)^(+)CTL频率、表型、分泌细胞毒性分子,HPV DNA水平。结果CA患者CD_(8)^(+)CTL、CD45RA^(-)CD27^(-)效应CTL、CD45RA^(-)CD27^(+)中枢记忆CTL、CD45RA^(+)CD27^(+)初始性CTL、CD45RA^(+)CD27^(-)效应CTL等CTL频率与表型高于对照组(P<0.05),而复发组患者上述CTL频率与表型均高于初发组(P<0.05);CA患者颗粒酶B、穿孔素、自杀相关因子配体等CTL分泌细胞毒力分子高于对照组(P<0.05),而复发组患者CTL分泌细胞毒力分子水平高于初发组(P<0.05);初发组与复发组HPV DNA低载量患者CTL频率与表型、CTL分泌细胞毒力分子水平低于高载量患者(P<0.05);初发组、复发组患者CTL频率与表型比例、CTL分泌细胞毒力分子水平与HPV DNA载量呈正相关(P<0.05)。结论外周血HPV16抗原特异性CD_(8)^(+)CTL表型及其分泌细胞毒性分子水平与尖锐湿疣发生、病情转归关系密切,同时其水平变化还与HPV DNA水平存在相关性。OBJECTIVE To investigate the phenotype characteristics of human papillomavirus(HPV)antigen-specific CD_(8)^(+) cytotoxic T lymphocytes(CTL)in peripheral blood of patients with primary and recurrent condyloma acuminatum(CA)and their relationship with HPV DNA.METHODS A total of 90 patients with CA admitted to Haikou People′s Hospital from Oct 2017 to Oct 2020 were selected as the study subjects,and divided into 43 cases of initial-onset group and 47 cases of the recurrent group according to the incidence of CA.Thirty patients who underwent circumcision operation or uterine fibroid surgery were included in the control group.The frequency,phenotype and secretion of cytotoxic molecules of HPV16 antigen-specific CD_(8)^(+) CTL,and HPV DNA levels were measured and compared between the two groups.RESULTS The frequencies and phenotypes of CTL including CD_(8)^(+) CTL,CD45 RA-CD27^(-) effect CTL,CD45 RA-CD27^(+) central memory CTL,CD45 RA^(+)CD27^(+) initial CTL,and CD45 RA^(+)CD27^(-) effect CTL were significantly higher in patients with CA than those in the control group(P<0.05),and the frequency and phenotype of the above-mentioned CTLs in the recurrent group were significantly higher than in the initial-onset group(P<0.05).CTL secreting cytotoxic molecules such as granzyme B,perforin and factor associated suicide ligand in patients with CA were significantly higher than those in the control group(P<0.05),while the levels of CTL secreting cytotoxic molecules in the recurrent group were significantly higher than those in the initial onset group(P<0.05).In the initial-onset group and recurrent group,patients with low HPV DNA load had significantly lower CTL frequency and phenotype as well as significantly lower levels of secreted cytotoxic molecules than patients with high HPV DNA load(P<0.05).The CTL frequency and phenotype,and the levels of secreted cytotoxic molecules were positively correlated with HPV DNA load in both the initial-onset group and recurrent group(P<0.05).CONCLUSION The phenotype of HPV16 antigen-spec
