摘　　要：目的 探讨孕妇感染B族链球菌(GBS)血清分型及阴道分泌物白细胞介素-6(IL-6)、基质金属酶8(MMP-8)、磷酸化信号转导及转录活化因子3(STAT3)表达水平。方法 选择2018年7月-2020年8月于新乡市中心医院产科进行产检的320例孕妇作为研究对象,根据GBS筛查情况分为GBS阳性组及GBS阴性组,检测GBS菌株血清型,比较两组阴道分泌物IL-6、MMP-8、磷酸化STAT3表达水平,并随访妊娠结局。结果 320例孕妇中GBS阳性48例,GBS阳性率为15.00%;基因分型以Ⅲ型、Ⅰa型为主,占比分别为64.58%、22.92%。GBS阳性组孕妇阴道分泌物IL-6、MMP-8、磷酸化STAT3表达水平均高于GBS阴性组(P<0.05)。GBS阳性组羊膜炎、早产、新生儿感染发生率均高于GBS阴性组(P<0.05)。结论 GBS阳性孕妇存在阴道分泌物IL-6、MMP-8、磷酸化STAT3水平的升高,GBS血清型以Ⅲ型占比最高,推测这一分型可能与不良妊娠结局的发生有关。OBJECTIVE To explore the serotypes of group B Streptococcus(GBS) and expression levels of interleukin-6(IL-6), matrix metalloenzyme 8(MMP-8), phosphorylated signal transducer and activator of transcription 3(STAT3) in vaginal secretions of pregnant women. METHODS A total of 320 pregnant women who received prenatal examination in obstetrics department of Xinxiang Central Hospital from Jul 2018 to Aug 2020 were recruited as the study subjects and were divided into the GBS-positive group and the GBS-negative group according to the result of screening of GBS.The serotypes of GBS strains were detected.The expression levels of IL-6, MMP-8 and phosphorylated STAT3 in vaginal secretions were detected, and a follow-up was conducted for pregnancy outcomes. RESULTS Among 320 pregnant women, 48 were tested positive for GBS, with the positive rate of GBS 15.00%;type Ⅲ and type Ia were the major genotypes, accounting for 64.58% and 22.92%, respectively.The expression levels of vaginal secretion IL-6, MMP-8 and phosphorylated STAT3 of the GBS-positive group were higher than those of the GBS-negative group(P<0.05).The incidence rates of amnionitis, premature birth and neonatal infection were significantly higher in the GBS-positive group than in the GBS-negative group(P<0.05). CONCLUSION The levels of vaginal secretion IL-6, MMP-8 and phosphorylated STAT3 of the GBS-positive pregnant women are elevated.The type Ⅲ is dominant among the serotypes of GBS, indicating that this serotype may be associated with the incidence of adverse pregnancy outcomes.
