摘 要:目的探讨黑皮质素-1受体(MC1R)基因多态性与烧伤后创面感染易感性及免疫功能的关联。方法选择新乡市中心医院2019年10月-2020年9月收治的烧伤后创面感染患者65例为感染组,选择同期医院收治的烧伤后未感染患者60例为未感染组。采用限制性片段长度多态性聚合酶链反应检测外周血MC1R基因rs1805005位点基因型,免疫磁珠法检测外周血CD_(4)^(+)、CD_(8)^(+)及NK细胞含量。结果感染组CC基因型、C等位基因频率高于未感染组,差异具有统计学意义(P<0.05),感染组TT、TC基因型频率与未感染组比较,无统计学差异;Logistic回归分析结果显示携带MC1R基因rs1805005位点CC基因型、C等位基因是烧伤患者发生创面感染的独立危险因素(P<0.05);烧伤创面感染患者携带TT、TC、CC型整体CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)比较差异具有统计学意义(P<0.05),但自然杀伤细胞(NK)比较,无统计学差异;CC型CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)低于TT型及TC型(P<0.05);TC型与TT型CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)比较,无统计学差异。结论MC1R基因rs1805005位点显著增加本地区人群烧伤后创面感染易感性,并对其免疫功能有一定影响,其机制有待进一步研究。OBJECTIVE To explore the association between melanocortin-1 receptor(MC1 R)gene polymorphism and the susceptibility to burn wound infection and immune function.METHODS Totally 65 patients with burn wound infection who were treated in Xinxiang Central Hospital from Oct 2019 to Sep 2020 were assigned as the infection group,meanwhile,60 burn wound infection without infection were chosen as the no infection group.The genotypes at rs1805005 locus of MC1 R gene in peripheral blood were detected by using restriction fragment length polymorphism polymerase chain reaction,and the contents of peripheral blood CD_(4)^(+),CD_(8)^(+) and NK cells were detected by means of immunomagnetic bead method.RESULTS The frequencies of CC genotype and C allele of the infection group were significantly higher than those of the no infection group(P<0.05),there were no significant differences in the frequencies of TT genotype and TC genotype between the infection group and the no infection group.The result of logistic regression analysis showed that the CC genotype and C allele at rs1805005 locus of MC1 R gene were the independent risk factors for the wound infection in the burn wound patients(P<0.05).There were significant differences in the overall CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) among the burn wound infection patients carrying with TT,TC and CC genotypes(P<0.05),there was no significant difference in the natural killer cells.The levels of CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) of the CC genotype patients were significantly lower than those of the TT genotype patients and the TC genotype patients(P<0.05);there was no significant difference in the levels of CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) between the TC genotype patients and the TT genotype patients.CONCLUSION The rs180500 locus of MC1 R gene may remarkably increase the susceptibility to wound infection in the burn wound patients and has certain impact on the immune function,and the mechanisms need to be further studied.
