摘  要:目的 探讨乙型肝炎病毒(HBV)感染孕产妇程序性细胞死亡因子1(PDCD-1)基因多态性与宫内感染易感性的关联。方法 选择2017年10月-2020年10月于新乡市中心医院分娩的乙型肝炎孕产妇120例,根据孕产妇新生儿是否发生宫内感染,将其分为感染组37例和非感染组83例,通过医院患者管理信息系统调取产妇及新生儿临床信息。新生儿出生后抽取其外周静脉血对血清中乙肝标志物:乙型肝炎表面抗原(HBsAg)、乙型肝炎表面抗体(HBsAb)、乙型肝炎e抗原(HBeAg)、乙型肝炎e抗体(HBeAb)、乙型肝炎核心抗体(HBcAb)及HBV-DNA进行检测,采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)检测PDCD-1基因rs2227981、rs2227982位点基因分型。感染组孕妇根据PDCD-1基因rs2227981不同基因型分为TT组和CT/CC组。结果感染组PDCD-1基因rs2227981位点TT基因型频率、T等位基因频率高于非感染组,差异具有统计学意义(P<0.05),两组rs2227982位点基因型、等位基因分布频率比较,无统计学差异;感染组孕产妇rs2227981不同基因型新生儿乙肝标志物模式及HBV-DNA水平比较,无统计学差异;感染组孕产妇rs2227981不同基因型新生儿出生时阿氏评分比较,差异有统计学意义(P<0.05),但两组新生儿体质量、新生儿溶血性贫血、新生儿感染性肺炎发生率比较,无统计学差异。结论 本地区HBV感染孕产妇发生HBV宫内感染可能与PDCD-1基因rs2227981位点突变有关,但与rs2227982位点无关。OBJECTIVE To explore the relationship between programmed cell death factor 1(PDCD-1) gene polymorphism of pregnant women with hepatitis B virus(HBV) infection and susceptibility to intrauterine infection. METHODS A total of 120 pregnant women with hepatitis B who gave birth in Xinxiang Central Hospital from Oct 2017 to Oct 2020 were enrolled in the study and divided into the infection group with 37 cases and the non-infection group with 83 cases according to the status of intrauterine infection in neonates. The clinical data of the pregnant women and neonates were collected by the hospital patient management information system, the peripheral venous blood specimens were drawn from the neonates so as to detect serum hepatitis B markers including hepatitis B surface antigen(HBsAg), hepatitis B surface antibody(HBsAb), hepatitis B e antigen(HBeAg), hepatitis B e antibody(HBeAb), hepatitis B core antibody(HBcAb) and HBV-DNA, the genotypes at rs2227981 and rs2227982 loci of PDCD-1 gene were detected by means of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP). The pregnant women of the infection group were divided Into the TT group and the CT/CC group according to the genotypes at rs2227981 locus of PDCD-1 gene. RESULTS The frequencies of TT genotype and T allele at rs2227981 locus of PDCD-1 gene were significantly higher in the infection group than in the non-infection group(P<0.05). There were no significant differences in the frequencies of genotypes and alleles at rs2227982 locus between the two groups. There were no significant differences in the patterns of hepatitis B markers and HBBV-DNA level among the neonates of the pregnant women with different genotypes at rs2227981 locus in the infection group;there was significant difference in the Ahri score of the neonates of the pregnant women with different genotypes at rs2227981 locus in the infection group(P<0.05).
