摘 要:目的 探讨过氧化氢酶(CAT)基因多态性与婴幼儿呼吸道合胞病毒(RSV)感染毛细支气管炎易感性的关联。方法 选择新疆生产建设兵团医院儿科2019年6月-2020年7月因下呼吸道感染所致毛细支气管炎入院治疗患儿为研究对象,根据患儿呼吸道标本病原学诊断结果,从中选择RSV感染患儿50例、非RSV感染患儿50例分别纳入RSV组和非RSV组,选择同期医院体检健康儿童50名为对照组。RSV组患儿根据病情严重程度分为重症组和轻症组,根据RSV病毒载量分为高载量组和低载量组,采用聚合酶链式扩增反应(PCR)检测CAT基因rs1001179位点基因分型。结果 RSV组CC基因型、C等位基因频率高于非RSV组和对照组,TT基因型、T等位基因频率低于非RSV组及对照组(P<0.05);非RSV组与对照组CAT基因rs1001179基因型、等位基因频率比较,无统计学差异;重症组CAT基因rs1001179位点CC基因型、C等位基因频率高于轻症组(P<0.05);高载量组CAT基因rs1001179位点CC基因型、C等位基因频率高于低载量组(P<0.05)。结论 CAT基因rs1001179位点CC基因型显著增加婴幼儿RSV感染所致毛细支气管炎易感性,且与病情加重及病毒载量升高有关。OBJECTIVE To explore the association between the catalase( CAT) gene polymorphism and the susceptibility to respiratory syncytial virus( RSV) infection-induced bronchiolitis in infants. METHODS The infants who were hospitalized in pediatrics department of Xinjiang Production and Construction Corps Hospital due to respiratory tract infection-induced bronchiolitis were recruited as the study subjects. According to the result of etiological diagnosis of respiratory tract specimens, 50 infants with RSV infection were assigned as the RSV group, and 50 infants without RSV infection were assigned as the non-RSV group, meanwhile, 50 healthy infants who received physical examination were chosen as the control group. The infants of the RSV group were divided into the severe group and the mild group according to the severity of disease, and the genotypes at rs1001179 locus of CAT gene were detected by means of polymerase chain reaction(PCR). RESULTS The frequencies of CC genotype and C allele of the RSV group were significantly higher than those of the non-RSV group and the control group, the frequencies of TT genotype and T allele of the RSV group were lower than those of the non-RSV group and the control group(P<0.05). There were no significant differences in the frequencies of the genotypes and alleles at rs1001179 locus of CAT gene between the non-RSV group and the control group. The frequencies of CC genotypes and C allele at rs1001179 locus of CAT gene were significantly higher in the severe group than in the mild group(P<0.05);the frequencies of CC genotype and C allele at rs1001179 locus of CAT gene were significantly higher in the high load group than in the low load group(P<0.05). CONCLUSION The CC genotype at rs1001179 locus of CAT gene may remarkably increase the susceptibility to the RSV infection-induced bronchiolitis, and it is associated with the aggravation of the disease and the increase of viral load.
