摘 要:目的:探讨计算机认知矫正治疗联合运动治疗对慢性精神分裂症患者的信息处理速度及血清BDNF水平的影响。方法:96例伴有认知功能损害的慢性精神分裂症患者被随机分入对照组、运动组及联合治疗组,在治疗基线、治疗第4周末及第8周末采用连线测验A和精神分裂症简易认知评估:符号编码测验对所有受试者进行评估并测定血清BDNF水平。结果:(1)与基线相比,联合治疗组在第4、8周末的连线测验A及符号编码测验的表现持续改善且血清BDNF水平持续升高(P<0.01);(2)与对照组比较,联合治疗组患者第4、8周末的连线测验A及符号编码测验及血清BDNF出现具有统计学意义的差异(P<0.05);(3)运动组患者仅表现出血清BDNF的持续升高(P<0.01),但连线测验A及符号编码测验未发现具有统计学意义的改善。结论:短期的CCRT联合运动治疗可提高慢性精神分裂患者的血清BDNF水平,并且改善该类患者的信息处理速度。Objective:To explore the effects of CCRT combined with physical exercise on the information processing speed and serum BDNF expression in patients with chronic schizophrenia.Methods:Ninety-six patients with chronic schizophrenia were randomly divided into control group,physical exercise(PE)group and combined treatment(CT)group.Information processing speed,which were evaluated using the Trail Making Test(TMT):Part A,and Brief Assessment of Cognition in Schizophrenia:Symbol Coding(BACS SC),and serum BDNF expression of all subjects were determined on baseline,the end of the 4rd and 8th weeks.Results:(1)Compared with baseline,the performance of TMT and BACS SC of patients in CT group continued to improve and the serum BDNF levels of patients in this group showed a trend of increasing at weeks 4 and weeks 8(P<0.01).(2)Compared with control group,there were significant differences in the TMT,BACS SC and serum BDNF of the patients in the CT group on weeks 4 and weeks 8(P<0.05).(3)However,patients in PE group only showed a continuous increase in serum BDNF(P<0.05).No significant improvement was found on the performance of TMT and BACS SC.Conclusion:CCRT combined with physical exercise may improve the information processing speed and increase the serum BDNF expression in patients with schizophrenia.
