摘　　要：目的分析2型糖尿病(T2DM)患者成纤维细胞生长因子-21(FGF-21)、骨钙素(OC)、羧化不全骨钙素(ucOC)变化情况及与血脂代谢指标的关系。方法选取2019年3月至2020年9月承德医学院附属医院收治的72例T2DM患者纳入T2DM组,选取同期95例体检健康的人员纳入健康组,比较两组血脂代谢指标总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和FGF-21、OC、ucOC水平。根据是否合并血脂代谢异常将T2DM组分为合并组及未合并组,比较两组FGF-21、OC、ucOC水平,并分析上述因子与血脂代谢指标的相关性。结果T2DM组TG、TC、LDL-C、FGF-21水平明显高于健康组,HDL-C、OC、ucOC水平明显低于健康组,差异有统计学意义(P<0.05)。T2DM组中,30例合并血脂代谢异常(合并组),42例未合并血脂代谢异常(未合并组)。合并组FGF-21水平明显高于未合并组,OC、ucOC水平明显低于未合并组,差异有统计学意义(P<0.05)。相关性结果显示,FGF-21与TC、TG呈正相关,OC、ucOC与TG、TC呈负相关(P<0.05)。结论FGF-21、OC、ucOC在T2DM患者中呈异常表达,且与T2DM患者血脂代谢异常密切相关,上述因子对T2DM发生及病情监测具有重要意义。Objective To analyze the changes of fibroblast growth factor-21(FGF-21),osteocalcin(OC)and incomplete carboxylated osteocalcin(ucOC)in patients with type 2 diabetes and its relationship with lipid metabolism.Methods 72 T2DM patients admitted to the Affiliated Hospital of Chengde Medical College from March 2019 to September 2020 were enrolled into the T2DM group,95 healthy persons who underwent physical examination dueing the same period were selected as the health group.The blood lipid metabolism indexes[total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)levels],FGF-21,OC,ucOC levels between the two groups were compared.The T2DM group was divided into the combined group and the non-combined group according to whether there was abnormal lipid metabolism,and FGF-21,OC,ucOC levels between the two groups were compared,and the correlation between the above factors and lipid metabolism indexes was analyzed.Results The levels of TG,TC,LDL-C and FGF-21 in the T2DM group were significantly higher than those in the healthy group,while the levels of HDL-C,OC and ucOC were significantly lower than those in the healthy group,and difference was statistically significant(P<0.05).In the T2DM group,30 patients had abnormal blood lipid metabolism(combined group),and 42 patients had no abnormal blood lipid metabolism(non-combined group).The level of FGF-21 in the combined group was significantly higher than that in the non-combined group,and the levels of OC and ucOC were significantly lower than those in the uncombined group,and the difference was statistically significant(P<0.05).The correlation results showed that FGF-21 was positively correlated with TC and TG,while DO and ucOC were negatively correlated with TG and TC(P<0.05).Conclusion FGF-21,OC,and ucOC are abnormally expressed in T2DM patients,and are closely related to abnormal blood lipid metabolism in T2DM patients.The above factors are of great significance to the occurrence and disease monitoring
