妇炎舒胶囊联合抗菌药物治疗慢性盆腔炎的疗效及对血清GM-CSF、MCP-1、TNF-α的影响

摘　　要：目的:探讨妇炎舒胶囊联合甲硝唑与左氧氟沙星治疗慢性盆腔炎(chronic pelvic inflammatory disease,CPID)的疗效及对血清单核细胞趋化蛋白-1(monocyte chemotactic protein-1,MCP-1)、粒细胞-巨噬细胞集落刺激因子(granulocyte-macrophage colony stimulating factor,GM-CSF)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的影响。方法:将本院(2020年3月~2021年7月)收治的CPID患者84例,按照随机数字表法分为观察组和对照组,每组42例。对照组给予妇炎舒胶囊模拟剂+甲硝唑+左氧氟沙星治疗,观察组给予妇炎舒胶囊+甲硝唑+左氧氟沙星治疗。比较两组疗效、中医证候积分和腰骶胀痛、小腹疼痛、带下量多消失时间;治疗前后血清MCP-1、GM-CSF、TNF-α水平。结果:治疗后,观察组总有效率(95.24%)高于对照组(78.57%,P<0.05);与对照组相比,观察组腰骶胀痛、小腹疼痛、带下量多消失时间短,主要症状和次要症状积分低,具有统计学差异(P<0.05);两组GMCSF、MCP-1、TNF-α水平均较治疗前下降,且观察组GM-CSF、MCP-1、TNF-α水平低于对照组(P<0.05)。结论:妇炎舒胶囊与甲硝唑、左氧氟沙星联用治疗CPID在减轻机体炎症反应、症状改善和临床疗效方面优于单纯使用抗菌药物,且未增加不良反应。Objective:To explore the clinical efficacy of Fuyanshu capsule combined with metronidazole and levofloxacin in the treatment of chronic pelvic inflammatory disease(CPID)and its effect on monocyte chemotactic protein-1(MCP-1),serum granulocyte-macrophage colony stimulating factor(GM-CSF),and tumor necrosis factor-α(TNF-α).Methods:A total of 84 patients with chronic pelvic inflammatory disease treated in our hospital from March 2020 to July 2021 were randomly divided into observation group(n=42)and control group a(n=42).Patients in the control group were treated with Fuyansu capsule simulator combined with metronidazole and levofloxacin,patients in observation group were treated with Fuyanshu capsule combined with metronidazole and levofloxacin.The clinical efficacy,traditional Chinese medicine syndrome scores,the extinction time of lumbosacral swelling pain,lower abdomen pain,excessive amount of leucorrhea,and the serum levels of MCP-1,GM-CSF,and TNF-αwere compared between the two groups before and after treatment.Results:After treatment,the total effective rates of the observation group(95.24%)was significantly higher than that of the control group(78.57%,P<0.05);compared with control group,the extinction time of lumbosacral swelling pain,lower abdomen pain and excessive amount of leucorrhea in observation group were shorter,cardinal symptom and minor symptom scores were lower,there were statistically significant between two groups(P<0.05);levels of GM-CSF,MCP-1 and TNF-αin the two groups were decreased compared with those before treatment(P<0.05),and the levels of GM-CSF,MCP-1 and TNF-αin the observation group were lower than those in the control group(P<0.05).Conclusion:Fuyanshu capsule combined with Levofloxacin and Metronidazole can reduce inflammatory reaction,improve clinical symptom,and enhance curative efficacy in the treatment of chronic pelvic inflammatory disease better than antibiotics alone,and do not increase the adverse reactions.
